SUPPLY OF ONE TYPE I MODULAR AMBULANCE
FOR WESTERN HEALTH

Invitation to Tender for One Type I Modular Ambulance
1.0 General Provisions
1.1 Intent
This invitation to Tender is intended to obtain One Type I Modular
Ambulance for the Western Regional Integrated Health Authority (Western
Health) at the Western Memorial Regional Hospital.
This Tender is concerned with the acquisition of One Type I Modular
Ambulance for the Western Memorial Regional Hospital with consideration
of the following:
- Ongoing service and maintenance support.
- All manuals, documents and initial supplies.
- The right to reproduce any printed materials supplied with the
product for the purpose of using the product.
- Training and training manuals.
- Future enhancement availability.
1.1.1 Western Health reserves the right to order additional units at the
same price for a period up to and including 31 December 2008.
1.2 Client Background
Western Health was established in 2005 and is responsible for the
delivery of Health and Community Services in the Western Region.
1.3 Vendor Response
1.3.1 Vendor’s tender must contain an Executive Summary which shall
contain:
a. A brief description of the product being quoted.
b. The name, title and address of the Vendor’s representative
responsible for the preparation of the Tender.
1.3.2 All prices quoted for goods and services must be specified in
Canadian dollars, FOB Western Memorial Regional Hospital. All
Tenders will be held to be valid for ninety (90) days following the
Tender closing date.
1.3.3 Tenders must be received in full on or before the exact closing time
and date indicated. TENDERS RECEIVED AFTER THAT TIME
WILL NOT BE CONSIDERED.
1.3.4 All costs relating to the work and materials supplied by the Vendor
in responding to this Invitation to Tender must be borne by the
Vendor.
1.4 Release of Information
1.4.1 While Tender is Open:
The names of individuals or companies who have picked up the
tender documents will not be released.
1.4.2 At Tender Opening:
Only the names of the bidders will be read out.
1.4.3 After Tender Opening:
1. No further information will be released until after the contract
is awarded.
2. After award, only the name and bid price of the successful
bidder will be made available.
3. Information will be made available for a 90 day period only.
1.4.4 FYI, Statements that are included as part of our Tender calls:
While bidders are welcome to attend the public opening, please be
advised that it is not our policy to release bid information. Only the
names of the bidders will be released.
1.5 Communication During Tendering
1.5.1 All communications with Western Health with respect to this
invitation to Tender must be directed in writing to the attention of:

1.5.2 Western Health may, during the assessment period, request
meetings with the Vendors to clarify points in the Tender. No
changes by the Vendor will be permitted after the Tender closing
date.
1.5.3 Faxed Tender responses will be accepted with the condition that
the original Tender documents are received at Western Health’s
Materiels Management Department no later than Five working days
following the Tender closing date.
1.5.4 All bids must be sent in a sealed envelope clearly marked with
Tender Name and Number to: Materiels Management
Department, Western Health, Western Memorial Regional Hospital,
Lower Level, P.O. Box 2005, Corner Brook, NL A2H 6J7.
1.5.5 Bids submitted by electronic transmission (e-mail) will not be
accepted.
1.5.6 Companies submitting fax Tenders are doing so at their own risk
and the fax Tender must be at the public opening as specified in
the Tender information. This Authority will not be responsible for in-
house courier services if companies submit quotations by fax
machine. The time stated on the fax Tender will become null and
void since it is the responsibility of the company placing the Tender
to have their Tender at the public opening, therefore, this Authority
will not be responsible for any damages or liabilities.
1.5.7 In order to contribute to waste reduction and promote
environmental protection, the Western Health will endeavour to
acquire goods and services that support these principles, therefore,
product(s) quoted should include:
maximum level of post-consumer waste and/or recyclable
content
minimal packaging
minimal environmental hazards
maximum energy efficiency
potential for recycling
disposal costs
must not reduce the quality of the product required or affect
the intended use of the product
must not significantly impact the acquisition cost
1.6 Tender Acceptance
1.6.1 Any acquisitions resultant from this invitation to Tender shall be
subject to the Public Tendering Act.
1.6.2 The Tenders shall be opened in the Private Dining Room at The
Western Memorial Regional Hospital on the scheduled date and
time.
1.6.3 Any Tender may be accepted in whole or in part. The lowest
Tender may not necessarily be accepted and Western Health
reserves the right to cancel the Tender call. Western Health shall
not be held responsible or liable for the payment of any costs that
are incurred by the bidder in preparing a Tender in response to this
invitation to Tender.
1.7 Warranty
The Vendor shall warrant that the product supplied to Western Health
shall equal the published specifications.
2.0 Product Specifications
2.1 Shall have 161.5” Wheelbase.
2.2 GVWR shall be 12,000 lbs.
2.3 Shall have 8 cylinder, 6.0 litre SFI V8 Vortex gas engine.
2.4 Shall have electronic 6 speed automatic transmission with overdrive.
2.5 Shall have 3.73 axle ratio with wide track axle.
2.6 Shall have locking differential.
2.7 Shall be a 4-wheel drive.
2.8 Shall have air ride suspension system for the rear axle with air dump.
2.9 Shall have manual shift transfer case with floor mounted shifter.
2.10 Shall have four wheel antilock disc brakes.
2.11 Shall have seven (7) highway BSW tires LT225/75R17. (Spare provided)
2.12 Shall have left and right outside black folding manual mirrors.
2.13 Shall have driver and front passenger air bag restraint.
2.14 Shall have front single zone air conditioning.
2.15 Shall have 40/20/40 split bench seat with dual manual seat back recliners, centre
fold-down armrest with open storage in cloth.
2.16 Shall have 160 ampere alternator.
2.17 Shall have chrome front bumper and molded plastic grille with chrome surround.
2.18 Shall have intermittent windshield wiper system.
2.19 Shall have AM/FM stereo with see/scan feature, digital clock, (4) speakers.
2.20 EXTERIOR BODY
Dimensions
2.20.1 Exterior box dimensions shall be approximately: 140" long x 84" high x
94”wide.
Modular Box
2.20.2 The modular body shall be all aluminum that is welded into an integral
structure.
2.20.3 The corner posts and edges shall be special die extruded ¼” thick.
2.20.4 Aluminum panels shall fit into grooves in the extrusion.
2.20.5 Shall use aluminum tubing structural members.
2.20.6 All door openings shall be extrusion framed.
2.20.7 The module exterior skin shall be of 0.100” aluminum single sheet
welded to the framing substructure and continuously welded at all
corner extrusions.
2.20.8 The panels shall be adhered to the structure tubing with double sided
tape and adhesives.
2.20.9 The drip rail shall be an integral part of the extrusion around the roof
perimeter.
Vehicle Doors
2.20.10 The cab doors shall be supplied with the chassis.
Exterior Doors
2.20.11 Shall have two man-door openings, one on the right forward side and
double rear doors for loading into the patient compartment.
2.20.12 The rear doors shall be double swing-out with capabilities of opening to
approximately 180 degrees.
2.20.13 All body doors shall use effective compression cellular type seals to
prevent leakage of dust, water and air.
Exterior ALS Access
2.20.14 Shall have an exterior door in the forward curb side of the body which
allows access to the contents of the ALS cabinet without entrance to
the patient compartment.
2.20.15 The door shall have compression cellular type seals to prevent leakage
of dust, water and air.
Exterior Backboard Compartment
2.20.16 Shall have an exterior compartment positioned at the rear street side
location of the body to house backboards and other patient handling
equipment.
Exterior Spare Tire Storage
2.20.17 Full size spare tire shall be located in an exterior compartment on the
street side. A panel with slam latch fastener shall cover the spare tire.
Windows
2.20.18 Windows shall be included on all doors, excluding compartment doors.
2.20.19 Rear doors shall have one window per door and the glass shall be
tinted and/or frosted.
Mirrors
2.20.20 Shall have right and left outside swing out mirrors.
Rear Step Bumper
2.20.21 Shall have an aluminum rear step bumper running the full width of the
body.
2.20.22 Shall have a door width flip up open grate step.
Mud Flaps
2.20.23 Shall have mud flaps for all wheels.
Striping
2.20.24 Stripe package shall be supplied in a reflective material. Mid Band
Stripping material shall be supplied in “Reflexite”. Rear stripping
chevron design shall be 3M (or equivalent). Reflective material
lettering, “Star of Life” and 911 decals shall be in reflective material.
2.20.25 Interior door surfaces shall be fitted with red reflectors.
Rustproof / Undercoating
2.20.26 The entire vehicle shall be undercoated with warranty.
Running Boards
2.20.27 Aluminum diamond plate located at cab rocker panels, running from
front wheels to modular body.
Side Step
2.20.28 Shall have a “U” type step for the side body entrance door.
Air Ride Suspension System
2.20.29 Shall have (1) Air Ride suspension system for the rear axle with air
dump.
2.21 PATIENT COMPARTMENT
Patient Compartment Interior Dimensions
2.21.1 The length of the patient compartment shall be approximately 124".
2.21.2 The width of the compartment shall provide approximately 48” of clear
aisle between the left wall cabinets and the squad bench.
2.21.3 The height of the patient compartment shall be approximately 66".
Floor
2.21.4 The floor covering shall be high quality seamless non-skid flooring
cemented to the sub floor.
2.21.5 The joints where the floor and cabinets meet shall roll up 90 degrees to
prevent seepage of fluids under the cabinets.
2.21.6 The floor of the drivers' compartment shall be covered entirely with
rubber matting.
Insulation
2.21.7 Cavities between the interior and exterior body paneling and doors
shall be filled with fire retardant spun fiberglass insulation.
Head Protection
2.21.8 Padding shall be placed at head area obstructions in the patient
compartment.
Door Panels
2.21.9 Shall have fiberglass door panels with access panels for latches for all
patient compartment doors.
Cab Rear and Body Front Wall
2.21.10 Shall have two sliding windows placed in the rear cab window and in
the front of the patient compartment to facilitate visible and sound
communication between the patient and driver’s compartment. A
flexible seal shall be installed between the windows.
Cabinets
2.21.11 The cabinets shall be constructed of industrial grade plywood covered
in formica and plastics.
2.21.12 A cabinet shall be fitted to the left side of the patient compartment from
floor to ceiling.
2.21.13 Storage compartments shall use sliding safety glass doors equipped
with “D” handles.
Squad Bench
2.21.14 Shall have a full-length squad bench with seat cushion and backrest
that accommodates three seated passengers or can accept a
secondary stretcher.
2.21.15 Shall have three seat belts.
2.21.16 Shall have three nylon restraining straps for securing a secondary
patient/stretcher.
2.21.17 Bench shall be equipped with two hold closed devices. A two-piece
squad bench top, with a hold open device, lifts for storage of
equipment.
ALS Cabinet
2.21.19 Shall have three shelves positioned on the curb side bulkhead for
storage of portable equipment.
2.21.20 The equipment shall be accessible from outside the vehicle from the
side loading door or a separate exterior compartment door and within
the patient compartment area.
2.21.21 Shall have an aluminum roll-up door securing the items inside the
cabinet inside of the patient compartment area.
Front Overhead Cabinet
2.21.22 Shall have a cabinet over the front bulkhead with sliding plexiglass
doors and a separate lockable cabinet.
IV Fluid Heater Storage
2.21.23 Shall have an IV fluid storage area in the main left wall cabinet.
Trash and Sharps
2.21.24 Shall have a trash container and sharps container located in the front
section of the squad bench.
Emergency Medical Technician Seat (EMT Seat)
2.21.25 The EMT seat shall be a bucket type seat located at the head of the
primary patient facing rearward.
CPR Seat
2.21.26 Shall be a side facing seat incorporated in the left wall.
Seat Belts
2.21.27 The squad bench shall have three lap belts. The EMT seat and CPR
seat shall have a three point seat belt restraint system.
Crash net
2.21.28 Shall have a removable crash net at the head of the squad bench.
Stretcher Fasteners
2.21.29 Shall have a cot securing device installed.
2.21.30 Shall have a Stair Chair – Stryker Model 6252 or equivalent.
IV Holders
2.21.31 Shall have two Heavy-duty double fold down anti-sway IV hooks with
velcro straps attached to the ceiling.
IV Pump Rail
2.21.32 Shall have (1) 24” IV pump rail installed on the main cabinet wall.
Grab Rail
2.21.33 Shall have a stainless steel grab rail on the ceiling over the primary
patient area.
Fire Extinguishers
2.21.34 Shall have a 5lb fire extinguisher located in the patient compartment
and a second 5lb extinguisher located in the cab.
Clock
2.21.35 Shall have an analogue clock installed on the bulkhead.
Coat Hooks
2.21.36 Shall have (2) coat hooks in the cab area behind each seat.
2.22 LIGHTING
Side & Rear Emergency Lighting
2.22.1 Shall have six flashing Super LED style warning lights located on the
perimeter of the ambulance, two facing rear and two each side.
Light Bar
2.22.2 Shall have a six position linear LED light bar positioned in the front
area, with four red and two clear light heads.
Traffic Advisor
2.22.3 Shall have an amber LED Traffic Advisor bar mounted over the rear
doors.
Grill Lights
2.22.4 Shall have two Super LED grill lights located on the front facing
forward.
Intersection Lights
2.22.5 Shall have four Super LED intersection lights mounted on each front
fender, two clear and two red.
Wig-Wags
2.22.6 Headlamps shall flash alternately in a wigwag fashion.
Mirror Lights
2.22.7 Shall have two Super LED mirror lights located on each mirror bracket.
Flood Lights
2.22.8 Shall have two angled LED style floodlights installed on each side of
the ambulance and over the rear doors of the ambulance.
2.22.9 The rear floods shall be activated when the rear doors are open, or
when the vehicle is placed in reverse.
2.22.10 The right side shall be activated when the side doors are open.
2.22.11 Switches in the driver’s console shall activate the lights.
Ambulance Interior Lighting
2.22.12 Shall have four fluorescent lights that can be controlled to low or high
intensity.
EMT Light
2.22.13 Shall have a small light located in the action panel.
Cabinetry Lighting
2.22.14 Shall have a switch in the EMT panel to activate lights in the cabinets.
Spotlight
2.22.15 Shall have one hand held spotlight that is hardwired in the cab.
Map Lights
2.22.16 Shall have two goose neck reading lights. One located in the action
area, the other on the cab console.
Fog Lights
2.22.17 Shall have fog lights mounted on the front bumper.
2.23 ENVIRONMENTAL
Exhaust Fan
2.23.1 The power exhaust vent shall be located in the main cabinet wall of the
patient compartment for the discharge of stale air.
Patient Compartment Heating and Air Conditioning
2.23.2 The patient compartment shall have a combination heater/air
conditioning unit with controls accessible in the action area.
2.23.3 Unit shall be ducted to vents in the patient compartment.
110 Volt Heater
2.23.4 Shall have a 110-volt heater in the rear of the patient compartment.
2.24 ELECTRICAL SYSTEM & COMPONENTS
Dual Battery System
2.24.1 Shall have two identical 12-volt heavy-duty battery systems. The
primary system shall power the entire vehicle. The second battery
system shall be for emergency starting only, and shall be activated
with a momentary switch in the cab.
Voltmeters
2.24.2 Shall have (2) volt meters in the cab console.
Cab Console
2.24.3 The cab console, containing switches and two-cup holders, shall be
mounted in the dash area. The console shall be easily removable.
Additional Switches – Cab Console
2.24.4 Shall have (3) additional switches to control patient compartment dome
lights, exhaust fan and heat/air conditioning.
Patient Compartment Controls
2.24.5 The EMT console and switch panel shall be located in the action area
next to the EMT seat.
Master Switch
2.24.6 Shall have a switch in the cab console to turn off electrical power to all
ambulance conversion circuitry.
Aerial Ground Plane & Radio Cables
2.24.7 Shall have a hole in the body ceiling to give access to an aerial base
and shall be pre-wired for mobile radio and GPS locating device.
2.24.8 Shall have power leads in the cab console.
12 Volt DC Outlet
2.24.9 The patient compartment shall be furnished with two 12-volt outlet
receptacles; located in the action area.
12 Volt DC Outlet
2.24.10 Shall have a 12V incubator type outlet receptacle located in the patient
compartment.
Door Open Light
2.24.11 Shall have an indicator light in the dash which illuminates when an
exterior door is open.
Back Up Alarm
2.24.12 Shall have a back up alarm located on the vehicle undercarriage and
shall activate when the vehicle is placed in reverse.
2.24.13 Shall have an auto reset “cut-off” switch in the cab console.
110-Volt AC External Power (Protected)
2.24.14 Shall have an external receptacle with Auto Ejection System to supply
electrical power to one 110-volt hospital grade outlets in the patient
compartment.
2.24.15 Shall have an engine block heater.
2.24.16 Shall have a 20ft long power cord.
2.24.17 The entire system shall be 30amp GFI protected.
Inverter
2.24.18 Shall have an inverter, 120 volt DC 10 110 volt AC, capable of
providing 2500 watts of continuous power.
2.24.19 Shall have one duplex receptacle located in the action area.
2.24.20 The shoreline shall automatically power the outlets, but when
unplugged, the inverter shall provide power.
Power Door Locks
2.24.21 The patient compartment doors (except spare tire door) shall have
power doors locks, activated with the door locking switch in the cab
and a hidden switch in the grille.
Keyless Entry System
2.24.22 Shall have a keyless entry system to lock and unlock all power locking
doors.
Battery Charger
2.24.23 Shall have one10 amp battery charger / conditioner.
Anti-theft System
2.24.24 Shall have a switch in the cab console that allows removal of the
vehicles ignition key while keeping the engine running. This shall lock
the steering wheel and prohibit changing the transmission from the
PARK position.
Code Red
2.24.25 Shall have a code red light installed in the cab console controlled by a
switch located in the EMT panel.
Auto Throttle
2.24.26 Shall have engine RPM to increase engine speed while the vehicle is
parked at idle.
2.24.27 Shall have an alarm sound to remind the driver to depress the park
brake, thus activating the auto throttle.
2.25 SIREN
Siren & Public Address System
2.25.1 Shall have a combination electronic siren with integral public address
system located in the cab console.
Dual Speaker System
2.25.2 Shall have a dual speaker system mounted forward facing with twin
100-watt drivers.
2.26 OXYGEN & SUCTION
“M” Tank Storage
2.26.1 Shall have storage for (1) “M” size oxygen cylinder with access through
the ALS compartment door.
“D/E” Tank Storage
2.26.2 Shall have storage for (2) “D” or “E” oxygen cylinders positioned at the
centre area of the front bulkhead wall, mounted vertically with ABS
cover.
Oxygen Outlet
2.26.3 Shall have one oxygen outlet in the action area and one over the
squad bench area.
2.26.4 The oxygen outlet shall be piped to the “M” size cylinder storage area.
On-Board Suction
2.26.5 Shall have an electric suction system located in the action area.
2.26.6 The collection jar shall be mounted vertically in the action panel.
Oxygen Equipment
2.26.7 Shall have one regulator for an “M” tank, one “M” tank wrench and flow
meters for the oxygen outlets.
3.0 Presentation / Training / Service
3.1 Presentation
A presentation of the Tender and / or a demonstration of the product /
system shall be provided, if requested, at the Vendor’s expense. Delivery
of the vehicle to be completed no later than March 15, 2007.
3.2 Training
The Vendor shall provide on-site in-service to staff. All costs associated
with this in-service shall be included in the total Tender price.
3.3 Service
3.3.1 The Vendor shall confirm in writing that Parts and Labour will be
available for the quoted system for not less than nine (9) years after
the warranty period.
3.3.2 The Vendor shall provide all Service and Parts manuals required to
service the equipment.
4.0 Product History and Vendor Reputation
4.1 The Vendor shall provide a list of three (3) organizations where a similar
Unit has been installed. Include a contact person for each organization.
5.0 Financial Considerations
5.1 All applicable taxes shall be indicated in the Tender.
6.0 Vendor Confirmation (please sign)
I confirm that our Tender meets or exceeds the specifications detailed in this

quoted price. Any items that are optional are noted accordingly.
