Sealed Tenders are invited from the Registered Suppliers/ Distributor/ Firms for supply of
Drugs and Consumables/ Instruments and Equipments to Malkangiri District for the year (2010-11)
the rate of tender will be valid for one year or till inviting of next tender which ever is earlier. The
tender papers in details along with the term and conditions are available in the office of CDMO,
Malkangiri (NRHM Section) and can be obtained on payment of Rs. 500/- ( Rupees Five hundred)
only as demand draft towards cost of tender papers on any working day within office hours. Bidders
can download the tender papers form the website:- www.malkangiri.nic.in / www.nrhmorissa.gov.in
and such Bidders have to enclosed a non-refundable demand draft of Rs/500/- (Rupees Five
hundred) only in favour of CDMO, Malkangiri with BID document and subscribe on the BID document
that it has been downloaded from the website. The sealed tender should reach to the undersigned on
or before Dt.27.08.2010 by 3.00 P.M.
Tenders will be opened on the same day after 3.00 P.M. in the office chamber of the CDMO,
Malkangiri.

TERMS AND CONDITION
1. The Tender will be received by the undersigned on or before dt.27.08.10 upto 3 P.M and will be
opened on dt.27.08.10 at 3.00 P.M. In presence of the tenders or their authorized representative. If
any bidder fails to be present there is no bar for opening of tender.
2. The tenderer can obtain tender papers by depositing demand draft of Rs. 500/- (Rupees five
hundred) only towards cost of tender paper in any working day within office hours of the Institution
from NRHM Section. Bidders can download the tender papers from the website:-
www.malkangiri.nic.in/ www.nrhmorissa.gov.in and the Bidders have to enclose a non-refundable
demand draft of Rs.500/- (Five hundred only) in favour of CDMO, Malkangiri with the Bid document.
3. The tender paper should be clearly typed / written in English and rates should be clearly visible.
4. The Specification of the Instruments and Equipments is to be mentioned clearly with original
catalogues and authorization of manufacturer with ISO/ISI standardization certificate is to be attached
in the tender paper.
5. Delivery of the Drugs/Instruments & Equipment should be FOR destination i.e. Central Store, DHH,
Malkangiri.
6. The rates should be mentioned inclusive of all taxes.
7. The undersigned/ Committee is not bound to accept the lowest tender rate considering the technical
aspect.
8. The unit of the items and name of the manufacturer should be mentioned clearly.
9. The rate of present tender will be valid up to 1 year or inviting of next tender which ever is earlier.
10. If any information or documents furnished by the tenderer are found to be incorrect or misleading at
any stage the tender will be rejected.
11. The Selected firm should obtain Test report of all items of Medicines before supply and sample
should be placed to DD, SDMU (O), Quality Section, Bhubaneswar for quality test.
12. The selected firm should deposit 2% of purchase order value towards processing fees and quality
testing fees at office CDMO, Malkangiri.
13. The Interested firm should deposit Rs.10000/- (Rupees Ten Thousand) only through SBI, Bank Draft
in favour of CDMO, Malkangiri towards earnest money with the tender paper which will be refunded
after finalization of tender process, otherwise tender will not be entertained.
14. The authority/ Committee reserve the full right to accept in full or reject any or all the tender without
assigning any reasons thereof.
15. The tenderer shall furnish the following documents with the tender paper.
i. Original money receipt for purchase of tender paper/ demand draft of Rs.500/-(wherever
applicable).
ii. Xerox copy of the valid Sales Tax (Vat) and Income Tax Clearance Certificate.
iii. Earnest Money Deposit (EMD) in shape of SBI, BD duly pledged in fovour of CDMO,
Malkangiri.
