MHSAA FOOTBALL FINALS PHOTO PASS REQUEST FORM

The building principal or athletic director shall use this form to request photo passes you will need in the
event your team advances to the finals of the MHSAA Football Playoffs, providing the names of the
individuals who will use the passes. This form must be received in the MHSAA Office by Noon on
Friday, Nov. 21. No requests for passes will be granted after that time.
Your normal team video camera operator(s) will be accommodated separately.
Still and motion images taken by individuals using these passes may only be used for school district
publications and promotional uses – these passes may not be used for photography for commercial
purposes. The MHSAA will not issue credentials to commercial photographers which have been at your
games leading up to the Finals.
By requesting and accepting these passes, the school and the photographers listed agree to those
conditions.
Please make the decision as to who will use these passes carefully, and let other people who may
have been taking pictures at your games know that you are limited to only two still photographer’s
credentials, and one videographer credential. Do not refer individuals to the MHSAA office to
attempt to secure additional credentials.
