Introduction
The Southern Health and Social Care Trust is one of the five new
Trusts established on 1 April 2007 under the Review of Public
Administration (RPA) replacing:
    Armagh & Dungannon Health and Social Services Trust,
    Craigavon Area Hospital Group Trust,
    Craigavon & Banbridge Community Health and Social
      Services Trust, and
    Newry & Mourne Health and Social Services Trust

The Southern Health and Social Care Trust delivers health and
social care   services to the population through four main
Directorates:

      Children‟s Services,
      Mental Health and Disability,
      Acute Services, and
      Older People and Primary Care

The Promoting Well Being Department sits within the Directorate
of Older People and Primary Care but works across all
Directorates and Programmes of Care. The Promoting Well Being
Department is managed by Melanie McClements, Assistant
Director for Promoting Well Being and brings together the
Community Development and Health Promotion Teams from the
legacy Trusts with the Southern Area Health Promotion
Department and Community Sector Training. This staff resource
has been reshaped with the appointment of a Head of Service for
Health Improvement, a Head of Service for User Involvement and
Community Development, the establishment of 3 locality
Promoting Well Being Teams and the creation of 2 Directorate
Leads – one to promote health and well being across Trust
Directorates and one to work within the Acute sector promoting
healthy hospitals.

The flow chart overleaf illustrates the new structures within
Promoting Well Being from September 2008.

Role                      As Head of Health Improvement, one of my main responsibilities is
                          to ensure that the overarching health improvement plans that have
                          been agreed with the Southern Health and Social Services Board
                          are delivered as per commissioning specification.

                          I link closely with locality managers and teams and in particular
                          with health promotion specialists to oversee the planning,
                          implementation and monitoring of these plans.

                          In addition, I will be providing leadership in the outworking of the
                          Trust‟s Promoting Wellbeing strategy which will engage a broad
                          range of partners and stakeholders within the Trust and across all

Role                       To support the implementation and ongoing development of the

                           work closely with the Southern Health and Social Services Board
                           Carers Strategy Group and other relevant organisations that are

Main areas of work         Developing and implementing the Trust Strategy.
                           Support the work streams of the Trust reference group. Ensuring
                           that the needs of carers are integral to the development of any
                           action plans. To ensure carer involvement of any planning of
                           new initiatives to improve the quality of life for carers.

                           To design information resource materials to address the needs of
                           carers.
                           To design a range of programmes to help support the needs of
                           carers.
                           To support and facilitate the design of initiatives such as a Carers
                           Mentoring Scheme, Carers Clinic within the hospital site.

                           To Work on how information on carers is monitored and
                           evaluated. To develop appropriate feedback mechanisms. To
                           maintain effective records and evaluate value for money of new
                           of existing schemes.

                           Deliver a professional service to carers in a professional and
                           competent manner.
                           To publicise and promote the Carers Strategy within local
                           communities, partners and stakeholders within the Southern
                           Health and Social Care Trust area.
                           To review the Carer Action Plan to ensure emerging needs are
                           met.

Wider Team contribution Through my needs assessments with carers any emerging needs
                        which present which link in to other Promoting Wellbeing streams
                        of work will be identified and information passed on to the
                        relevant worker in this area. E.G Sexual Health needs of young
                        carers.
                        I feel that carers will benefit from all areas of work within the
                        Promoting Wellbeing team and will work closely with all staff
                        members to ensure that carers needs are catered for when
                        drawing up Promoting Wellbeing Team Action Plans.

Community Development Workers

“ Community Development is about the strengthening and bringing
about change in communities…It is a way of working which seeks
to encourage communities… to tackle for themselves the problems
which they face and identify to be important.. which aim(s) to
empower them to change things by developing their own skills,
knowledge and experience, and by working in partnerships with
other groups and statutory agencies”

   Community Development Workers have a dual role:
       o Within the Community to provide:
              Support for organised community activity, and
              Support for local community development
       o Within the Trust to provide:
              Support for organisational development, and
              Support for staff training and development.
   Community Development Workers work with a range of

Community Sector Training
Mission statement:
“To improve health and well being by sharing expertise through
community education”
    Since 1997 CST has delivered Child Protection and good
      practice training to the community sector using 25
      community-based trainers who are trained, supported and
      monitored by the project.
    Since 2003, CST has also provided Community Development
      and Health Issues training
    CST has a management group/partnership involving statutory,
      voluntary and community representatives and locality
      reference groups which have helped to define how the project
      works

Traveller Support
   Provide sensitive and practical support to Travellers using a
    community development approach to improve health and
    social well being
   Increase Traveller participation
   Networking and partnership working
   Encourage appropriate use of services
   Raise awareness of particular health and social well being
    issues
