Project Reporting Requirements
Questionnaire
The Project Reporting Requirements Questionnaire is used to determine the project's reporting requirements - to whom
does the project need to be reported (Internal or State) and in what capacity (Major or Non-Major). The Project Complexity
Questionnaire (used in conjunction with this questionnaire determines WHAT the documentation requirements are for the
project.
Instructions: Please complete each of the lightly shaded boxes below. There are drop down responses for each of the five
questions. All five question must be answered for the worksheet to properly identify the project classification.
Please save your results to your computer while completing the Project Proposal Process. You can upload this spreadsheet
to the Project Workspace after the proposal is saved and published by selecting a document library in the workspace and
clicking the 'Upload' button.
