Dear Mr. Mazzulla,
Please find attached the application for a well location variance, as well as various e-
mails between myself and Susan Rabideau at the Rhode Island Department of Health
regarding the variance.
Please review the variance carefully, as some of stipulations contained therein will
require the Department of Parks and Recreation to keep records which detail when the
fertilizer is applied, how much was applied, who applied it, etc. as well as keep current MSDS
sheets. All of these stipulations are required by the Department of Health (see e-mails).
If everything looks good on the application, please sign the variance and return it to
the Department of Health in the enclosed envelope. Please also keep a copy on file for your
own records. Also, if you know what type of fertilizer will be used on the athletic fields,
please attach that information to the variance application as well.
I still have not heard back from the Planning Board regarding my request to either get
the two lots combined or get an easement for the well. I will contact them today to see what
is going on with this.
Also, I understand that Rosciti Construction has already installed the sewer line on the
site. If a portion of this sewer line is within the 200 foot well radius, I will try to work this out
with the Department of Health so that the line does not have to be re-installed.
