                              FOR STUDENT ENROLLMENT AND PARTICIPATION
                                                DHE 06


  I.       INSTITUTIONAL IDENTIFICATION

           Complete all institutional identification items at the top of the form. Provide the name and telephone number of th
           person who is responsible for completing this form.

 II.       REPORTING PERIOD

           Student data are to be reported annually in the fall for the current fiscal or academic year. Student data reported th
           fall will reflect the characteristics of first-time, degree-seeking undergraduates enrolled as of the institution’s curre
           academic year Fall census date.

III.       GENERAL INSTRUCTIONS

           This form requests the count of first-time, degree-seeking undergraduate students by different ranges of three com
           ability descriptors. Averages and counts of students for whom the particular measure is unknown or unavailable ar
           also requested.

           A.    CALCULATING CLASS RANK PERCENTILES

           Use the following formula to calculate class rank percentiles:
           (class size – student class rank + .5) / class size

           B.    CLASS RANK DECILES

           ―Highest 10%‖ = Class rank percentiles > 90.
           ―Second 10%‖ = Class rank percentiles > 80 and <= 90.
           Etc.

           C.    CALCULATING AVERAGES

           Average ACT Composite and Combined SAT Math and Critical Reading (Verbal) scores are requested. Please
           calculate these figures by taking the sum of the relevant scores divided by the number of students for whom this
           information is available. Do not count missing values as zeroes.

IV         DEFINITIONS

           First-Time Student (Undergraduate): A student who has no prior postsecondary experience (except as noted below
           attending any institution for the first time at the undergraduate level. This includes students enrolled in academic o
           occupational programs. It also includes students enrolled in the fall term who attended college for the first time in
           prior summer term, and students who entered with advanced standing (college credits earned before graduation fro
           high school; IPEDS).

           Full-Time Student: Undergraduate—A student enrolled for 12 or more semester credits , or 12 or more quarter cre
           or 24 or more contact hours a week each term. Graduate—A student enrolled for 9 or more semester credits, or 9 o
           more quarter credits, or a student involved in thesis or dissertation preparation that is considered full time by the
           institution. First-professional—As defined by the institution (IPEDS).

           Degree-Seeking Student: A student recognized by the institution as seeking a formal award at the reporting institut
           High school students also enrolled in postsecondary courses for credit are not considered degree/certificate-seekin
           (IPEDS).

           Freshman: A student who has completed less than the equivalent of 1 full year of undergraduate work; that is, less
           than 30 semester hours (in a 120-hour degree program) or less than 900 contact hours (IPEDS).
