• Erosion control maintained.
G. Work planned for the coming week:
• One more big load of roll beams is scheduled to be delivered tomorrow, Thursday,
March 22. All rolled beams are available with exception of rolled fascia beams.
Started steel erection last week with 7 iron workers. Rolled beams at Main Waiting
area on grid D, D.1, E, E.1, and E.2 are in place. Continue welding purlins between
the in-place beams. Some purlins are shorter need to be fill-in with shim pieces, some
purlins need to be trimmed. Fill-in are structurally fine. Will be ready for installing
curved deck by the end of next week.
• Continue rough in plumbing, copper line, insulation pipes, and VAV at the south side.
• Continue ductwork rough in at center and west areas toward the south side.
• Will start installing fire sprinkler next week.
• In process of installing sheet rock hard lids and soffits. Continue taping at the low roof
areas, second coats at Examine Rooms.
• Aluminum windows and glazing were completed. Replacements of broken glass will
be here with aluminum storefronts next week.
• Start installing exterior hollow metal door hardware.
• Emergency generator and MC panel were bumped to Friday, March 23 due to
needing to work on the concrete (stair, landing) at the northwest corner of the
building
• Stan Edwards talked to Mark of Questar Gas to bring the line to the meter, Ascent will
pour the meter pad next week. Ryan Henrie has a check to pay Questar; Questar will
release the work order.
• Americom (cables/wires installer) has caught up with installing cables.
• SLVHD will need to contact Quest for phone lines installation.
• Site concrete (walkway, pads) for the south side will be completed next week. Start
grading the north side.
• Start landscape irrigation.
• Sign contractor will put the footing for the monument sign.
• Continue maintaining dust and erosion control. Patched silt fence.

1. Unresolved Items from Previous Meetings:
11-15-1 Issue: Ascent emailed the update overall construction schedule to Huina Ho with Substantial
Completion Date on April 17, 2007. Brandt Knowlton explained that due to the bad
weather and change orders which have put the Substantial Completion in April, but
he thought that March 30, 2007 is a workable date. Ascent will rework on the overall
construction schedule and make the Substantial Completion Date on March 30, 2007,
so South Main can move in April 2007.
Update 11-22-2006: The updated overall construction was emailed to Huina Ho. The revised
Substantial Completion Date is April 3, 2007.
Update 11-29-2006: Ryan Henrie indicated 15 days extension will put the Substantial Completion
Date on March 29, 2007. Ascent will work toward this date.
Update 12-6-2006: Brandt Knowlton confirmed that there are only one 24' and one 20'
TS8x20x1/2 beam are available. There are only 2 mills in the north America
can roll the TS8x20. The mill will roll every 90 days. The steel submittal were
ready on August 8. The TS8x20 will be in production on December 22. After
TS being fabricated in Albana, they will be shipped to the roll shop at
Portland, Oregon. After being rolled, they will be shipped to the Utah Steel
Ornamental for preparation and then delivered to South Main job site. Ascent
will check and follow the dates of progress of curved beams process. Ascent
will look into different scenarios of construction schedule. (i.e., to isolate the
curved beams areas, work in the closed areas not to slow down by the curved
beams).
Update 12-13-2006: Brandt Knowlton is waiting for a letter from the mill to confirm the date to
update the schedule. Brandt said that there is one production date prior to
December 22 but we’ll not make the date when the submittal was ready on
August 8. Ascent will follow the steel tube progress then will know how this
will impact the overall schedule. Kristy Cottrell needs to know the Substantial
Completion Date in January 2007 in order to schedule patients for April
month.
Update 12-20-2006: Steel mill is scheduled to roll the steel tubes on Dec. 22. The Substantial
Completion Date has been changed to April 27, 2007 on the updated
schedule. Ryan Henrie asked if the updated schedule includes overtime and
double crews. Although the steel tube availability problem, this will not
change the Contractual Substantial Date.

Update 1-3-2007: a. Albina Mill at Oregon will spend 450 hours to roll and shape the tube
steel beams; the rolled beams are in the process of being shaped. The
rolled beams are scheduled to arrive in Salt Lake on the third week of
January.
d. Ascent needs to update the overall construction schedule to reflect the
delay of rolled steel beams. Ascent will add the high metal roof to the
schedule and change the date of low membrane roof.
e. Ricardo Flores said that SLCO Real Estate is checking possibility extend
one more month lease for the existing South Main building.
Update 1-10-2007: Ascent prepared a revised overall schedule. The revised schedule has April
27, 2007 as the Substantial Completion date. The schedule is contingent on
the rolled steel tube beams.
Update 1-17-2007: Furniture coordination meeting was scheduled on Wednesday, 10:00 a.m.,
Jan. 24. South Main can only close one week and not see patients. Thus, we
need to confirm the date for South Main to move.
Update 1-24-2007: a. Artwork Coordination (Day Christensen, Foundry) -
1. Artworks are completed and ready to be installed.
2. The date will be before flooring is in place to install artwork.
Day Christensen and Foundry will follow up the date with Stan
Edwards.
3. A push plate is on the entry walkway block wall and interfere
with the artwork. Nancy McKendrick will coordinate with Day
Christensen to either split 8 panels or install 6 panels at entry
walkway and 2 panels inside of bldg. Otherwise, a push plate
on a post will be added to the opposite side of CMU wall.
b. Furniture Coordination (Carmelle Jensen of CCG and Source One
furniture installer) -
1. CCG ordered the furniture to avoid the price increase, so the
furniture are manufactured and stored in Salt Lake.
2. When the furniture arrive in a trailer, it’s everything and not
by areas.
3. CCG will start installing furniture on April 2. It will take 2
weeks to install and 1 week to do the punch list. CCG will
follow up the date with Stan Edwards.
c. Phone/Data Coordination (Mark Monson of Americom) -
1. 3/4"i conduit can fit 4 wires. SLCO’s standard is one voice
plug and one data plug for phone/data outlet shown on the
E3.1. South Main will mark up the needs for terminations, so
Americom can revise the bid and give to Dave Hatt.
2. Americom wants to start pulling cables when ceiling grids are
in, and no ceiling tiles.
d. South Main moving date - South Main can start moving at the end of
April. South Main can close existing facility and move the computers
and equipment on May 14, 2007.
Update 1-31-2007: Day Christensen wanted the 8-panel artwork connected and centered to the
entry walkway CMU wall. Thus, the push plate will need to be mounted on a
pole.
Update 2-14-2007: a. Push plate will be mounted on a galvanized post, a detail is needed.
b. South Main schedules patients’ appointments 3 months in advance,
and South Main can only block out one week. Thus, Kristy Cottrell
needs to confirm the moving days either the week of May 14 or 21.
Ascent will update the overall construction schedule when the upper
roof beams are installed.

Update 2-21-2007: a. Ascent updated the overall construction schedule based on the upper
roof beams will be completed on March 16. Building will be
substantial completed on May 18. South Main may start move-in on
May 21. Ryan Henrie will talk to Kristy Cottrell to see if South Main
can move-in a week after May 21.
b. Ryan Henrie indicated that CCG & Howell charged $900 for storing
furniture until the middle of April, since the Substantial Completion
Date was moved to May 21; SLCO may have additional charge again,
Ryan will find out from CCG & Howell.
c. Installation of cable/wire will remain un-changed. Furniture and
Artwork installation date will be moved accordingly.
Update 2-28-2007: a. South Main will start moving at May 25, Memorial Day week, and
open on June 4.
b. Ascent wrote a letter to request a 90-day extension for 8x20 steel tube
delay and bad weather. Ryan Henrie stated that the bad weather (6.5
days), change orders (14 days), and steel tube delay (24.5 day) will
add up to 45 days, and he did not see a 90-day extension. He’ll be ok
to prepare a Change Order for 45 days time extension to put the
Substantial Completion Date on May 18.
Update 3-7-2007: Steve Forbes, commissioning agent, indicated that building will need to be
purged two weeks after Substantial Completion on May 18. Thus it’s better for
South Main to move-in on June 4. Kristy Cottrell will free up the appointments
on the week of May 21 and 28.
Update 3-21-2007: South Main will start move-in on June 11. Ricardo Flores will give a letter to
SLCO Real Estate to explain the extension of existing building lease from May
to July.
12-20-3 Issue: The smooth form finishes (exposed aggregates and honey combs) on the concrete
archway columns are not acceptable. Cement plaster patches will not be accepted.
Update 1-3-2007: the installer wants to try to patch honey combs on the archway columns when
the weather is not too cold.
Update 1-10-2007: The concrete installer used adhesive as bonding agent to patch the
honeycombs. The team will wait until the concrete cure next week to review
the patched columns.
Update 1-17-2007: Nothing has been done due to the cold temperature. Ascent will get the patch
material info for review.
Update 2-28-2007: Architectural Concrete started concrete column patch and repair yesterday.
The patch and repair work look very patchy and is not acceptable at this
stage.
Update 3-7-2007: Architectural Concrete indicated that they should use concrete pump not
bucket to pour concrete columns, that caused the aggregates separation.
Architectural Concrete will try to fix the uneven concrete finish by Monday,
March 12.
Update 3-14-2007: Stan Edwards said that Architectural Concrete has worked on patching and
repairing columns last week, they need one more week to complete the
patching and repairing.
Update 3-21-2007: Concrete patch and repair for the second column from north looks good.
Ascent will provide a warranty to ensure the patch will stay bond to the
concrete.
2-28-3 Issue: a. JI#15 - Since we cannot lock the egress Door No. 53 and 57 (install key pad).
Kristy Cottrell may want to re-evaluate the key pad control from public to
employee sides.

b. Ascent prepared a C.O.P. for $3,883.59 for JI #15.
Update 3-7-2007: South Main and Ryan Henrie will review the COP.
Update 3-14-2007: Ryan Henrie indicated that the C.O.P. is acceptable, he’ll add one additional
day for C.O.. King Cobra key pads for Immunization Storage are electronic
hardware and are not returnable. Thus, these two key pads will need to be
given to SLVHD for other use.
3-14-1 Issue: The light fixtures at the high rolling roof area will be hung from unistrut spanned
between the horizontal beams as shown on details.
3-14-2 Issue: The South Main building is Type II-B Construction (non-combustible, non-rated
construction), no fire rated walls are required. The building is setback 10' from the
north property line. Thus, the north wall is a non-rated wall and can have windows.
The CMU and brick wall are 2-4 hour fire-rated and don’t need to have the gypsum
board to be the fire-rated wall. The walls shall be constructed per Wall Types shown
on A1.1.
3-14-3 Issue: The rust metal roof deck at the Check-In area will need to be cleaned and repaired
per section 05310.3.5.
2. New Items:
3-21-1 Issue: a. Electrician wants to use MC cables for the light fixtures at the open area to
minimize the conduit bends.
b. Electrician asked if we need the 1 ½"i conduit for projector screen.
c. Electrician asked if the photo sensor at the open areas can be mounted to the
walls in lieu of ceilings.
3-21-2 Issue: Ricardo Flores asked Ascent to brush paint and not to spray paint for the exterior
steel.
3-21-3 Issue: Steve Forbes, commissioning agent, asked for the following items:
a. Training Syllabus/Outlines needs to be submitted for review.
b. The status of O & M Manual.
c. Proposal for equipment start up.
