NOTES FOR COMPLETION
1) Enter the project title in the 'Project Title' field, and the designated four-digit project code in the 'Project Code' field.
2) Provide the name of the Project Manager and the Project Leader in the 'Project Manager' and 'Project Leader' fields.
3) Provide concise details regarding the project objectives in the 'Objectives' field.
4) In the 'Deliverables' area, list all the items that are going to be produced as part of the project: consider hardware, software,
documentation, requirement specifcations, training materials, procedural documentation, tender documents, etc.
5) In the 'Milestone / activity' column, list all the milestones and activities that have to be undertaken to complete the proposed project.
Each milestone or activity should include the following information:
- 'Agreed Start':
Enter the most up-to-date start date for the activity that has been agreed by the Project Board (or Project Leader is there is no Project
Board). Remember to consider the end date of any activities that the activity is dependant upon.
- 'Agreed Effort (days)':
Enter the most up-to-date estimated number of days effort that are needed to complete this activity that has been agreed by the Project
Board (or Project Leader if there is no Project Board).
- 'Agreed End':
Enter the most up-to-date end date for the activity cthat has been agreed by the Project Board (or Project Leader is there is no Project
Board).
- 'Individual / team assigned':
Specify the team or individual that has been assigned to complete this activity. This information should only be entered once the Manager
responsible for the member of staff has been consulted and has confitmed the availability of the member of staff required.
- 'Forecast / actual Start':
A date should be entered here either if the start date for the activity has moved or is looking likely to move from the agreed start date.
Once the activity has actually started, the actual start date should be recorded here (even if it is the same as the agreed date).
- 'Forecast / actual End':
A figure should be entered here either if the required effort for a the acivity has changed or is looking likely to change from the agreed
effort. Once the activity has been completed, the actual effort should be recorded here (even if it is the same as the agreed effort).
- 'Forecast / actual End':
A date should be entered here either if the end date for the activity has moved or is looking likely to move from the agreed end date. Once
the activity has been completed, the actual end date should be recorded here (even if it is the same as the agreed date).
- 'Progress':
As the activity progresses, keep track of progress by regularly updating this field with a % completion. There is no way to provide an exact
figure for this field, but it is useful for reporting purposes to keep track of at least a rough % completion.

- 'Comments':
Use this field for any comments or notes regarding the actvity or milestone.
6) If any issues are encountered during the project, record them in the 'Issues' area. An Issue can be a problem that is holding up the
project, a general concern, or simply a question, and each Issue should include the following information:
- 'Status':
New - this is a new issue, and no action has been taken against it yet.
In progress - action is being taken to resolve this issue.
Escalated 1 - the issue cannot be resolved by the Project Manager, and has been escalated to the Project Leader or Project Board.
Escalated 2 - the issue cannot be resolved by the Project Board or Project Leader, and had been escalted to Senior Management or the IS
Board.
Resolved - the issue has been resolved.
- 'Description':
Provide details of the issue, concern or question.
- 'Owner':
Every issue must be assigned an owner - this is the individual who is responsible for ensuring the issue is resolved. It is the Project
Manager's responsibility to make sure the owner is aware they are responsible for the issue!
- 'Action':
Provide details of the action(s) being taken to resolve the issue, or what was done to resolve the issue.
7) If additional space is needed for issues, simply select the bottom row of the issues section and drag downwards. If additonal space is
required either for deliverables or activities, right click on on the left-most column of the relevant section and select 'Insert'.
