h e / s h e w o r k s f o r t h e c o m p a n y, p u r p o se a n d d u r a t i o n o f t r a v e l , i n c l u d i n g
signature with title, full name and seal of the company:
a) I n c a s e t h e a p p l i c a n t i s b u s i n e s s o w n e r o r l e g a l r e p r e s e n t a t i v e , t h e
employment letter should be signed by him/herself;
b) I n c a s e t h e a p p l i c a n t w o r k s f o r f a m i l y b u s i n e s s , t h e e m p l o y m e n t l e t t e r
signed by siblings is not acceptable;
c) I n c a s e t h e a p p l i c a n t i s e m p l o ye e , t h e e m p l o y m e n t l e t t e r s h o u l d b e
s i g n e d b y H R m a n a g e r.

16. For those whose Hukou and work place are not in the same province, 1
copy of residence permit issued at least 6 months and still valid after
expiration of the visa (Original needs to be presented during
interview)
17. For minors, certificate of family relationship (legalized by Foreign
A ff a i r s O ff i c e ) a s w e l l a s c o n s e n t l e t t e r o f p a r e n t s :
a) if the minor travels with both parents, the consent letter only needs to
be signed by both parents;
b) if the minor travels with one parent, the consent needs to be legalized

18. The form needs to be signed by the applicant in person in Chinese.
