SALES TAX INSTRUCTIONS
1. Go to www.pa100.state.pa.us to register your business and obtain a
Sales Tax License number. See attached instructions.
2. Complete PA Form Rev-1220 according to attached instructions
3. Send the following to Market America at P.O. Box 35364,
Greensboro, NC 27425:
a. PA Form Rev-1220
b. Market America Sales Tax Agreement form
c. Copy of your Sales Tax License sent to you by Pennsylvania –
mark on this license that this is the license for PA – apparently,
it does not look like other state’s licenses and this will prevent
them from coming back to you to ask for a copy.
