PPO Horizon Plans and Health Reimbursement Arrangements:

Combining Choice and Affordability

PPO Horizon Plans allow employees to fight the rising cost of healthcare by combining

the lower premiums of a higher deductible medical plan with the advantages of a health

reimbursement arrangement (HRA). An HRA provides tax advantages and encourages

employees to make responsible healthcare decisions. All plans include a prescription

drug benefit and offer attractive options such as maternity, Preventive Care Plus and

supplemental accident coverage.

Health Reimbursement Arrangements (HRAs)

An HRA is a defined contribution arrangement, funded solely by the employer, that

works in conjunction with a higher deductible medical plan. Employers use HRA

funds to reimburse their employees' eligible healthcare expenses. For employees, all

reimbursements from the HRA are tax-free. For employers, expenses paid from the

HRA are tax deductible. HRA funds belong to the employer, who decides how much

from any in-network or out-of-network provider. By selecting an in-network provider,

maximum for in-network services. The in- and out-of-network deductibles and out-of-

pocket limits accrue separately on these plans. All plans include a prescription drug

card and offer attractive options such as an encounter fee for in-network office visits,

maternity and alternative prescription drug card deductible choices.
