Knees In Need!
Patellofemoral Stress Syndrome
How many times have you heard your dance teacher tell you to “Stretch your knees in your
grand battement!” or “Bend your knees when you land your jump!” or “Soft standing leg while you
shuffle!”? A dancer’s knees are essential components of not only the line and style, but also of the proper
biomechanics of each dance movement. Proper biomechanics are very important to ensure proper
alignment of the leg to prevent injuries.
The knee joint is a hinge joint, which allows two principle movements: flexion (bending your knee) and
extension (stretching your knee). The knee joint complex consists of the femur, the tibia, the fibula, and
the patella (kneecap). The patella is the largest sesamoid bone in the human body – meaning it is a
bone inside a tendon. It is located in the tendon of the quadriceps femoris muscle. The patella sits in two
grooves on the femur and tracks up and down with movement of the knee. It is very important that the
patella stays within this groove, or else the dancer will experience pain and weakness. Tracking within
this groove depends on the pull of the quadriceps muscle and patellar tendon, the depth of the groove,
and the shape of the patella.
Patellofemoral stress syndrome results from the patella moving sideways as it tracks in the femoral
groove. An imbalance between the muscles around the knee joint can lead to this abnormal tracking of
the patella. Some examples of imbalances include:
• tight hamstrings and gastrocnemius (your calf muscle)
• tight in lateral retinaculum (the tissue that connects your quadriceps muscle to your patella)
• tight iliotibial band (a band of tissue running down the outside of your thigh)
• flat feet
• weak inner thigh muscles
• altered alignment of the pelvis, hip and knees
Signs and symptoms of patellofemoral stress syndrome are:
• tenderness on and around the kneecap
• swelling around the knee
• dull ache in the centre of knee
• pain when going up and down stairs
• pain with walking, jumping or running
• pain and stiffness with prolonged flexed knee postures like sitting or squatting
Proper treatment of patellofemoral stress syndrome should address the underlying
causes leading to the knee pain. For example, if the patella is tracking sideways
because of a tight iliotibial band and weak inner thigh muscles, treatment should
include stretching the iliotibial band and strengthening the inner thighs. Other
treatment strategies include modalities to treat pain and swelling (ultrasound,
interferential current, laser) and knee taping techniques. A registered physiotherapist
can assess the muscular imbalances, treat the pain and swelling, and provide an
individualized exercise program to help the dancer return to full, pain-free dancing!
