Directions for Obtaining a DEA Registration Number
1. GENERAL:
A. All Principal Investigators who need to use controlled substances in their research must
obtain a Drug Enforcement Administration (DEA) Registration Number and register with
the Texas Department of Public Safety (TDPS) for a state license. Apply for the DEA
number FIRST.
B. All Principal Investigators who need to obtain an application from the TDPS, please call
(512) 424-2188 or by e-mail at tppcsr@txdps.state.tx.us.
NOTE: For TDPS applications, Applicant signature must be in BLUE INK. And, in the left
margin write: “NEW”.
C. State employees are exempt from the license fee.
D. The Certifying Official is normally a department chair or high level member of the
administration who can certify that you are a state employee.
E. The DEA Registration process takes about 4 to 6 weeks and must be completed prior to
issuance of a state license. Once you register your laboratory may be site visited by DEA
personnel at any time to inspect your drug storage system and verify your compliance
with applicable Federal regulations.
F. The application program is self directing and easy to complete. Some directions are
provided below.
2. DEA REGISTRATION
A. Go to: www.deadiversion.usdoj.gov/drugreg/index.html to complete a New or Renewal
Application online and pay using a credit card. The current fee is $184.00 / year.

B. Or go to: http://www.deadiversion.usdoj.gov/drugreg/reg_apps/pdf_apps.htm to
download a PDF version of the form. Once completed you will print and mail the form to
the address on the form.
C. Choose DEA Form 225 for New Applications (Researcher)
D. SECTION 1: Drug Registration- Click on New Applications.
E. Next Page: read directions and click on DEA 225.
F. Next Page is start of Application Form. Do not click on researcher on this page unless you
need Schedule l drugs. Scroll to bottom of page and click on BEGIN.
G. On next page (1 Personal Page 1) begin completion of form: Put the room number where
drugs will be stored as part of the street address. Click on NEXT.
H. On (1 Personal Information Page 2) enter your social security number or tax ID number
and check “CERTIFICATION FOR FEE EXEMPTION” box or you will have to pay for
the license. Click NEXT.
I. On ( 1 Personal Information Page 3) Fill in:
1. First blank with UNTHSC.
2. Second Blank with your “certifying official name” (department chair, dean, or VP)
who will certify that you are a state employee.
3. Third Blank with his/her official title.
4. Fourth Blank with their phone number.
5. Check box regarding: “I HAVE READ THE ABOVE, AND AGREE” certifying
official’s consent to be named on application. If you don't, you won't be able to
proceed.
Click NEXT.
J. SECTION 2: Business Activity/Schedules:
1. Select Schedule(s )needed. Do not select Schedule 1 on this form. Select
Schedules 2, 3, 4, & 5 for drugs useful in research. If you select schedule II, you
will need to designate at another site on this form the drugs you will be
requesting.

NOTE: It is imperative that you order only what you will for all intents and purposes use
up in your laboratory procedures. Any controlled substances left over and / or expired must be
properly inventoried and “disposed” of according to the Federal regulations.
2. If you select Schedule II, you will need to check the box for official order forms,
which you must have to order Schedule II drugs.
3. Otherwise, select 3 (narcotic and non narcotic), 4, & 5, and do NOT check the box
for order forms.
4. Click NEXT and proceed.
K. SECTION 3: State licenses
1. For State License, leave blank unless you have an active professional license.
2. Choose “TX-TEXAS” for State License State
3. Mark State Control Substance License leave blank or enter your active license.
4. Click NEXT.
L. SECTION 4: Background Information
1. Answer questions.
2. Click NEXT.
M. SECTION 4: Drug Code Numbers -Schedule II
1. If you have selected Schedule II, you must make drugs selections now.
2. Make selections and click ENTER.
3. If you didn't select Schedule II, you won't see the drug selection lists and will
have passed on to SUMMARY OF INFORMATION.

N. SUMMARY OF INFORMATION.
1. Review Information and make any corrections needed.
2. READ WARNING.
3. Type FIRST and LAST NAME in block.
4. Push “SUBMIT BUTTON” to complete application process.
O. Receipt of Licenses
1. Provide a copy of your state and federal license to the Director of Research
Compliance.
