The Illness-Wellness Continuum
Key Concept #1: Wellness is a process, never a static state.
This model shows the relationship of the Wellness and Treatment Paradigms.
Moving from the center to the left shows a progressively worsening state of health.
Moving to the right of center indicates increasing levels of health and well-being.
The Treatment Paradigm can only take you to the neutral point, where the symptoms of disease
have been alleviated. The Wellness Paradigm, utilized at any point on the continuum, moves
one towards ever higher levels of wellbeing.
The Illness-Wellness Continuum
Think of the continuum as a pathway. People can be headed in either direction.
A person who is generally physically healthy, but who is always worrying about
their health and complaining, may be to the right of the neutral point, but may be
facing towards the left, in the direction of premature death.
A person who is physically or mentally challenged may have a positive outlook and
be cultivating love instead of fear, and consequently may be facing to the right, in
the direction of high-level wellness.

Taking the Assessment
Clients assess themselves in 12 key dimensions of wellness.
About the Assessment
There are 10 statements for each dimension - 120 statements total.
Each statement describes a wellness action, skill, belief, attitude or
awareness.
The assessment generally takes 30-45 minutes to complete.
If a client is unable to complete the assessment at one sitting, they can
save their results and login at a later time to complete it.

Creating My Wellness Plan
Creating My Wellness Plan
After completing the assessment and viewing their scores, your client can
choose to continue to create a Personal Wellness Action Plan or log off and
return to create their plan in a later session.
The Personal Wellness Plan enables a client to create 1-5 simple action steps by
following some basic guidelines.
A wellness coach can be very beneficial in creating a realistic wellness action
steps based upon the specific areas of wellness the client is most motivated to
Change (as revealed by their assessment results).
If the client chooses to log off and create their plan later, they will log in to
“My Wellness Homepage” where they can create their Personal Wellness Plan
(highlighted on next slide), and access other wellness tools including My
Wellness Journal, the Self-Study Center and the 12 Wellness Resource Centers.

Email Reminders – A Key Tool to
Support Your Wellness Journey
Your client will receive an email reminder containing their action
steps at the frequency requested when creating My Wellness Plan.
Email reminders help your client to stay focused on successfully
completing their action steps, and on building a sense of self-
efficacy in creating personal change.
Email Reminders – Providing Direct
Access to Update Your Action Steps
In addition to providing regular exposure to action steps, email reminders
also provide a direct link to the “Update My Wellness Plan” tool. This
direct link enables your clients to update their action steps without having
to login at www.MyWellnessTest.com.
Clients can also directly access My Wellness Homepage from their email
reminders without having to log in.

Email reminders also provide direct access to “My Wellness Tools”, helping
clients to be supported in successfully completing their action steps using the
Kaizen approach of small steps for continuous improvement.

Maximizing the Impact
of Email Reminders
To get the most of email reminders, clients can schedule a specific time on
the day(s) they are sent to take 5-10 minutes to focus on their action steps.
Using the links on their email reminder, clietns can check to see if they
need to adjust or edit any of their action steps. You can also use this time
to make an entry in My Wellness Journal, read a commentary in the Self-
Study Center in an area of current focus, or to visit the Wellness
Resource Centers.
Clients can access the full page color wellness wheel to focus on the
12 dimensions to bring a whole person perspective to their day.

You can access “My Wellness Tools” at the bottom of My Wellness
Homepage. To edit your action steps, click on “My Wellness Plan”.
From My Wellness Plan edit page your client can view their Wellness
Plan, edit their plan, or change the frequency of email reminders.
Updating Action Steps
To update their action steps, your clients can click on the “Edit” link
on My Wellness Plan. This will enable them to add, edit, or delete
action steps. They also have access to the tutorial, “Tips for Creating
Effective Action Steps” to help you refine their action steps.
Your clients will also be able to access the “Update My Wellness
Plan” page from a link in their email reminders.
As a licensee, you can edit your client‟s wellness plan on your Admin
Menu, on the User Homepage (access from the User Reports)

and insights and to track their progress in implementing action steps.
Clients select one of the 12 areas of wellness to make a journal entry.
Each entry is recorded and dated chronologically for ease of access
on future visits. Clients can access the Wellness Journal from My
Wellness Homepage or the link in their Email Reminders.
Excellent tool for succeeding in the Kaizen method of the small steps
for continuous improvement to support sustainable lifestyle change.
Clients automatically receive an email introducing them to My Wellness
Journal 10 days after completing their wellness plan.

The Self-Study Center contains expert commentaries on the 10
wellness statements from each of the 12 sections of the assessment
(120 commentaries total).
Clients can start by reading commentaries from the specific areas of
wellness in which they have created have action steps. This supports
clients in gaining a deeper understanding in these areas of their life.
Within each of the 12 areas the statements are listed within in the order
of the your client‟s level of motivation to change.

The Resource Centers have been created to provide helpful resources,
information, and tools to assist your clients in successfully implementing
their action steps and achieving a higher level of health and wellbeing.
The Resource Centers act as a “Wellness Solution” in all 12 dimensions
of wellness, enabling clients to access expert articles, commentaries,
exercises, and books, as well as other educational resources.
Clients automatically receive an email introducing them to the Self-
Study Center 24 days after completing their wellness plan (1 week after
the email reminder for the Self-Study Center).
Expert Resources for Your Clients
The 12 Resource Centers are a great resource for your clients. You
can direct your client to specific articles and other resources you feel
with support them in successfully implementing their action steps and
deepening their knowledge.
You also have access to the 12 Resource Centers from your client‟s
“User Homepage” in your administrative menu.

The Wellness Inventory can help you monitor your client‟s ongoing wellness,
and to become your client‟s year-round wellness solution. Use the program
to create a year-round engagement and relationship with your clients and to
help them develop a deeper commitment to wellness.
Reassessment & Monitoring Progress
Your clients can assess their level of wellness prior to their arrival and
again at 6 months. Then, monitor their progress with the Test
Comparison feature.
By re-subscribing clients after the first year for an additional year, they
will be able to reassess at 12 months and then again at 18 months,
allowing you to continue your relationship and your role as their
personal wellness solution. Clients can be renewed for multiple years.
