1、奥运会开/闭幕式门票购票人登记表（以下简称“登记表”）所收集的观众信息和照片，将用于制作门票和入场查验。请您仔细阅读本填 写说明并按要求正确填写。 form carefully. 2、请将填写完整并粘贴了照片的登记表与身份证件复印件一同交给北京奥组委指定的中国银行门票代售网点。仅限当面递交（本人或代 办人），不接受邮寄或营业性专送。 Please deliver the filled paper form with the photo, together with a copy of your ID certificate to any one of the BOC tickets selling outlets designated. Please deliver the documents yourself or by your proxy, post documents will not be accepted. 3、您填写的姓名和身份证件信息必须与您购票时使用的姓名和证件一致。中文姓名请使用简体中文书写；英文姓名请使用大写英文字母 书写。 The name and ID information filled in the form must be the same as that you used to purchase the ticket(s). Chinese name must be filled in with Simplified Chinese, for English name, please fill in with capitalized letters. 4、填写的国籍须与所持有效证件签发国家或地区一致。 The nationality filled in the form must be the same as the information shown on the ID. 5、通讯地址须填写包括国家或地区、省、市、区、街乡、楼、门牌号等全称信息内容。 The Mailing Address must contain detailed information, e.g., No., Building, Street, District, City, Province, Country/Region. 6、选择身份证件类型时请参照： Please refer to the following abbreviation of ID types.
