Outdoor Fall Soccer plays M-Th 3-7pm and Su 12-7pm. Indoor Spring Soccer plays M-Th 5pm-midnight and
Sun 5-10pm. Please fill out all 3 sections below. The intramural office will do their best to accommodate but
there are NO guarantees. Unless cancelled by the Intramural Office, games that are cancelled or forfeited will
not be rescheduled. Keep in mind that NO accommodations will be made for those on multiple teams.

Participants can only be on (1) team per league (Men’s, Women’s, Co-ed, and Open) unless the 2nd team is in
the Mid Division. (e.g. Ice Hockey High and Mid OR Mid and Low)
Participants can play on no more than two (2) teams per sport across all leagues. All participants must be on the
roster prior to participation. (e.g. 5v5 Basketball Men’s High and mid OR Men’s High and Co-ed)
