Please complete the relevant attached forms in UPPER CASE and send them with the correct fees to your appropriate Area Registrar at the address given overleaf. Cheques
should be made payable to the area e.g. Solent Area Association.
The Area Registrar will return one copy of the form to you together with the appropriate number of individual membership cards. If you are a Club Secretary please hand the
membership cards to each individual for signature and retention.
PLEASE NOTE: In order to receive Zone Press Online and ANY other EB electronic communication you must enter the EMAIL addresses of each individual. An individual's
registration should not be duplicated - e.g. if you have registered as a coach you do not need to register again to play - once is enough.
Any Active Coach/Official who intends to play as well must complete the COACH/OFFICIAL FORM to be covered by the Coach/official Insurance.

Club Fees Individual Membership Fees
• Senior Club £50.00 • A Senior player pays their annual membership (£10)
• Junior Club £35.00 • The insurance premium is paid to our insurers (£1.50)

• Associate £5.00 • The breakdown for an U12 player is (£0.50) Insurance, (£0.60) retained locally,

Individual Top Up Insurance

All Risks Club Insurance

Membership fees include an administration fee to England Basketball.
Clubs must register all members (including committee members) representing the club. INSURANCE COVER IS ONLY VALID WHEN PLAYING, COACHING OR OFFICIATING IN ENGLAND
Failure to do so could invalidate the policy cover. BASKETBALL AFFILIATED COMPETITIONS, GAMES OR COURSES.
