Y.     COMMON BIDDER PITFALLS
         1. EDGE Option A, B or C is not selected on the Commitment to Participate in the Edge
             Business Assistance Program form, which is part of the Bid Form.
         2. Bidder fails to indicate a percentage (%) when selecting option B on the Commitment
             to Participate in the Edge Business Assistance Program form.
         3. Attorney-in-fact signature on bond is illegible.
         4. Bidder does not circle the prime contracts (i.e. General, HVAC, Plumbing, Electrical,
             Etc.) that make up his combination bid.
         5. The “Contract Description” on the Bond is missing or does not match exactly what is
             being bid.
         6. Bidder is not enrolled in the OBWC Drug Free Work Place program.
         7. Company name on Bid Form does not match the Secretary of State Business filings.
         8. Company name on Bid Form does not match the Bidders license for HVAC, Plumbing,
             Electrical, Etc.
         9. Incorrect DMA form is used.
         10. EEO Certificate is expired or will soon expire.
         11. Workers Compensation Certificate is expired or will soon expire.
         12. Ohio Department of Insurance Certificate of Compliance (to support the bond) is expired.
         13. Bidder is not responsive to the time sensitive nature of the Bidder’s Qualifications
             Request or Notice of Intent to Award submittal requirements.
         14. Bidder does not provide a complete duplicate of their bid and bond on bid day.
         15. General Liability and Builders Risk Insurance certificate or coverage issues.
         16. Certificate of Good Standing with the Secretary of State is expired or unavailable.
         17. Bidders fail to complete the Sum in Words spaces on the bid form.
         19. Bidder fails to indicate if the Alternate is an “add” or “deduct”.
