FAQ for OCVMRC Recognition Awards
To apply for the 2011 OCVMRC Recognition Awards please go to this link:

If you have an account, please log in at this page. If you forgot your password, please select the option under the ‘GO’
button.
If you do not have an account please follow these steps:
1. On the page below you will see ‘CREATE ACCOUNT.’ Please select this option if you need to create an account.
2. You will need to enter your email address in the E-MAIL LOOKUP to see if there is an account associated with
your email address. Then you need to click ‘SEARCH.’
3. Once the search produces no results, you will need to click ‘CREATE NEW ACCOUNT.’
4. The page below will populate and you will need to enter your information. Once you enter your information,
please select ‘NEXT.’
5. You will come to the ORGANIZATION LOOKUP. Here you will need to add only your State and City in the search
field. Next you must select the ‘ORGANIZATION TYPE’ in the drop down menu. Then you need to select
‘SEARCH.’
6. A list of different organizations will appear and you must click ‘AFFILIATE’ with the organization. If the
organization is not listed, you will need to select ‘SEARCH AGAIN.’
7. Next you will need to select your role in the organization. Below are specific roles for someone associated with
an MRC unit. Once you select the role, you will need to click “CREATE AFFILIATION.’
8. Once you complete the following steps, you will need to save your profile and then move to the application.
9. You will want to select ‘NACCHO APPLICATIONS’ on the left side of the page to find the 2011 OCVMRC
Recognition Awards application.
10. Next you will want to select ‘2011 OCVMRC Recognition Awards.’
11. Once in the application, you will need to complete the required answers and on the last page, you must click
‘FINAL SUBMIT,’ on the page that says %100.
