					                                       GENERAL INSTRUCTIONS
This is a worksheet to help you define the key progress points you expect your organization or
participants to achieve, the steps you will take to get there, and the timing of when you hope each
milestone whill be achieved. Please review the example provided (last tab at the bottom on the
right) before completing your own.

You will need to "maximize" Excel for this to work, and for you to see all the tabs you require. Excel
should be "full-screen" - click the maximize button on the top right of Excel (PC) or top left (Mac) to
do so.




You should then see a series of tabs at the bottom left of your screen similar to those below.




Step 1 - Complete your Milestones & Timeline:
Regardless of how long you expect it will take for you to complete your project, implement your organizational
priorities or interact with your program participants, (6 weeks, 3 months, 1 year, 3 years), it is helpful to have
key progress points along the way. This allows you to “check in” on interim achievements and make course
corrections as necessary.

Click on the STEP 1 tab below to enter your milestones & timeline.

Step 2 - Complete your Project Steps
For each milestone you want the organization or those you serve to reach, there are activities, actions, and/or
services that you must undertake, complete or provide in order for that to happen. You need not articulate
every step you plan to take, but simply summarize the critical elements that much occur to achieve each
milestone. It's OK to combine several efforts for each milestone. Incorporate “If - then” logic: if we complete
these activities, then the organization or participants will accomplish these milestones."

Step 3 - Copy your Worksheet into the Application
This is the place that brings it all together for you. Simply copy and paste the contents here into the answer box
in the application form. You can also save this worksheet to your computer by using the File, Save prompts at
the top of the worksheet and selecting a drive on your local computer or network.

Click on the STEP 3 tab below to review your statements, then copy and paste your final answer.
