Tdap Requirement for Rising 6th graders
Since July 1, 2006, State law requires that all children entering the 6th grade MUST have a booster dose of Tdap
vaccine, if at least 5 years have passed since the last tetanus-containing vaccine. Tdap is the vaccine for Tetanus,
Diphtheria and Pertussis . Tetanus containing vaccines can be documented as DPT, Td, Dtap, or Tdap.
RISING 6TH GRADERS MUST HAVE THIS VACCINE BEFORE ENTERING SCHOOL IN
Please provide the school with documentation showing the vaccine has been received. This documentation can be
brought in or mailed into the middle school over the summer. (See address below)
We are required to EXCLUDE children from school until the needed documentation is provided.
The only exceptions to the law are listed below:
1. a religious exemption - which requires that a Religious Exemption Form be completed, notarized and
provided to the school, or
2. Documentation from a physician indicating the child cannot receive the vaccine due to medical reasons.
Having this done early will prevent disruption at the beginning of the school year, so schedule early.
Immunizations can be given at doctor’s offices or the local health dept. Please contact your school nurse or the
nurse at the middle school for questions or concerns.
