                                     Asbestos Abatement & Demolition of the
                                            former Dixie Square Mall,
                                        15301 Dixie Highway, Harvey, IL

The South Suburban Mayors & Managers Association (SSMMA) will receive bids for asbestos
abatement and demolition of the former Dixie Square Mall, 15301 Dixie Highway, Harvey, IL until 2:00
P.M., Central Standard Time on the 14th day of November, 2011, at 1904 W. 174th Street, East
Hazel Crest, Illinois 60429 at which time and place all bids will be publicly opened and read aloud.

Plans shall be available beginning October 7, 2011 from BHFX Digital Imaging. Bidders may
purchase specifications and plans by going to www.bhfx.net, clicking on "Enter Document
Management Center" which opens up the "Public Projects" list, then clicking on "Dixie Square Mall
Asbestos Abatement and Demolition Project". If you have any questions in regards to receiving

Mandatory Pre-Bid Meeting/Site Visit
There is a mandatory pre-bid meeting required for this project to be held on October 17, 2011, at 3:00
P.M. at the Ingalls Memorial Hospital Professional Building located at 71 W. 156th Street, Harvey, IL
60426 followed immediately by a mandatory visit to the site at 15301 Dixie Highway, Harvey, IL.
Bidders must attend the pre-bid walk-through, visit the site and sign in with Terracon at both locations
to be eligible to bid this project. (Please Note: Prior to entering the site, anyone showing up for the
site visit will need to have steel-tipped boots and a hard hat. Safety glasses and a reflective vest are
recommended.)

A certified check or bank draft, payable to the order of South Suburban Mayors & Managers
Association, or a satisfactory bid bond executed by the bidder and an acceptable surety in an amount
equal to five percent (5%) of the total bid shall be submitted with each bid.

Time for Completion: 182 calendar days after execution of the contract and receipt of the Notice to
Proceed.

The South Suburban Mayors & Managers Association reserves the right to reject any or all bids or to
waive any informalities in the bidding.

Bids may be held by the South Suburban Mayors & Managers Association for a period not to exceed
30 days from the date of the opening of bids for the purpose of reviewing the bids and investigating
the qualifications of bidders, prior to awarding of the contract.

Attention is called to the fact that not less than the minimum wages as set forth in the Contract
Documents must be paid on this project.

The successful bidder will be required to comply with Section 3 of the Housing and Urban
Development Act of 1968 (Section 3), which provides that employment and other economic
opportunities generated by certain HUD financial assistance shall, to the greatest extent feasible, and
consistent with existing Federal, State and local laws and regulations, be directed to low- and very
low-income persons.

In accordance with Title VI of the Civil Rights Act of 1964, the HUD regulations issued pursuant
thereto (24 CFR Part 1 – Nondiscrimination in Federally Assisted Programs of the Department of
Housing and Urban Development), and the Illinois Human Rights Act (775 ILCS 5), the SSMMA
hereby notifies all respondents that it will affirmatively ensure that minority business enterprises will
be afforded full opportunity to submit proposals responding to this invitation and will not be
discriminated against on the grounds of race, creed, color, religion, sex, marital status, sexual
orientation, familial status, national origin or ancestry, age, physical or mental handicap unrelated to
ability, military status, or unfavorable discharge from military service in consideration for an award
concerning any contract entered into pursuant to this advertisement. It is the goal of SSMMA
that 20% of the total value of the awarded contract, including any subcontracts issued pursuant
thereto, will benefit qualified southern Cook County contractors including MBE/WBE/DBE.

                                Section 3 Business and Employment Notice

South Suburban Mayors & Managers Association (SSMMA) is preparing to carry out the Asbestos Abatement
and Demolition of the Former Dixie Square Mall through the use of Community Development Block Grant
Funds.    In the implementation of this project the following job types may be available:

        asbestos abatement workers
        laborers (demolition)
        truck drivers
        security guards
        electrician
        plumber

All job openings will be posted at City of Harvey, City Hall – 15320 Broadway Avenue, Harvey IL. 60426 and
SSMMA - 1904 W. 174th Street, East Hazel Crest, Illinois 60429. To the greatest extent feasible, employment
and training positions will be made available to qualified low- and very low-income persons who permanently
reside in Southern Cook County. Persons qualified for the jobs listed should register at the following location:

                    City of Harvey City Hall, 15320 Broadway Avenue, Harvey Illinois 60426
                          SSMMA, 1904 W. 174th Street, East Hazel Crest, Illinois 60429
Additionally, the following contracts and procurements will/may be made and to the greatest extent feasible,
businesses located in Southern Cook County which currently employ or commit to employing local low- and
very low-income persons will be utilized:

        asbestos abatement services
        building demolition services
        site clearing and grading
        concrete crushing services
        special waste (ACM) hauling and/or disposal
        construction debris (non-ACM) hauling and/or disposal
        site security (guards, fencing)
        temporary utilities (sanitary facilities, trailers, storage)


All above-referenced procurements will be made on a competitive basis. The names of businesses who
respond to this notice will be included on procurement lists for this project. Names of qualified low- and very
low-income job seekers will be provided to contractors.
