We would love to shoot your upcoming portraits! Your session will include up to an hour of time. The creative
fee for this service is $75 and includes unlimited outfit changes. Following your session, a preview will be made
available on our blog within a few days (many times that same day!), and your finished proofs will be made
available on our website, password protected so that you may share with as many or as few friends as you’d
like. If you can’t choose just a few, take home a disk of all your images for just $25.

Maternity + Baby’s First Year $599 (Save over $140!)
Take advantage of this amazing special which includes a maternity session, newborn session within 14 days of
your son or daughter’s birth, as well as additional sessions at 3, 6, 9, and 12 months (Total sessions value -
$600). All sessions will include disk of images with print release. Also includes an 8x10 from each session
($60 total value), a 10-page 10x10 Layflat press book ($80 value) at the conclusion of the year, and 20% off any
design/invitation/announcement purchases for birth announcements or birthday invitations. (Total value $740+)

We require the use of a release form for all portrait sessions.
RELEASE
I hereby consent to the photographing of myself and the recording of my voice and the use of these photographs
and/or recordings singularly or in conjunction with other photographs and/or recordings for advertising,
publicity, commercial or other business purposes without compensation to me/us/my minor child. I understand
that the term "photograph" as used herein encompasses both still photographs and motion picture footage. I
further consent to the reproduction and/or authorization by Choice Media Productions/Adam Kruminas/Melissa
Kruminas to reproduce and use said photographs and/or recordings of my voice, for use in all domestic and
foreign markets.
I hereby release Choice Media Productions, and any of its associated or affiliated companies, their directors,
officers, agents, employees and customers, and appointed advertising agencies, their directors, officers, agents
and employees from all claims of every kind on account of such use.
I further release and hold harmless Choice Media Productions and its representatives from any claims of
personal injury or property damage while participating in these activities.
If Model is under 18: I am the parent/legal guardian of the individual named below, I have read this release and
approve of its terms.
