2. Job Purpose
To work as part of the North of England Cardiovascular Network to
improve the whole pathway of care for CVD patients and their carers.

organisations involved with care of cardiac patients. This will involve
project planning and management, developing excellent relationships
with all key stakeholders, coaching and facilitating the team in redesign
skills in order to develop and deliver a significant improvement in cardiac
services.
3. Dimensions
The North of England Cardiovascular Network has 14 full time staff and 3
clinical leads, 14 PCTs, 9 Acute Trusts, 3 Ambulance Trusts and 3
Strategic Health Authorities. The Service Improvement Manager needs

Ensure that the patient’s voice and the patient’s needs are at the heart of the

To work with staff in stakeholder organisations to promote best practice in
adopting the principles laid out in the Network’s PPI Strategy.
Support all organisations within the local health Community to deliver agreed
plans to improve services in line with the national and local CVD priorities.
Take a network wide lead in managing specific projects.
Advise on initiative bids in line with local priorities.
Working with staff in stakeholder organisations to provide an understanding of
the potential achievements of undertaking service improvement activities in the
North of England Cardiovascular Network.

Facilitate the identification of opportunities for service redesign to service
providers.
Challenge thinking regarding traditional ways of working and service
development/improvement.
Provide training and coaching for relevant staff groups in the development and
implementation of service improvement and change management.
Ascertain that work programmes and local standards are in line with current
thinking on best practice and research.
Facilitate the development of pathways of care that meet health needs and
deliver appropriate treatment across organisational and professional
boundaries.
Ensure that local improvement programmes have robust arrangements in place
to sustain service improvement beyond the life cycle of individual projects.
