PERMISSION AND RELEASE TO ATTEND FIELDTRIP EVENT(S)
I hereby give permission for my child to attend any scheduled fieldtrip(s) conducted by Roswell Budokan
during the week(s) my child(ren) attend Summer Enrichment Camp(s). I understand that during the
fieldtrip my child will be transported In a Roswell Budokan van.
The undersigned expressly agrees to assume any and all risks inherent in the transportation by vehicle of
my child, and incident to and from the fieldtrip location, and accept and assume all responsibilities for
losses, costs, personal injury and damage or loss of any kind or description resulting from any negligent
act or omission by any instructor, officer, director, employee, agent or member (collectively called
Members) of Aikido Association of Atlanta, Inc./Roswell Budokan or by any owner or lessor of the
premises known as Roswell Budokan, and expressly releases all such entities and individuals from any
liability arising from any negligent act or omission causing damage or injury to my child or myself.
The undersigned further agrees to indemnify and hold harmless all such individuals and entities from
any and all claims made or instituted against it or them which may arise out of my child’s participation in
this event. By and through this Release, if I, or anyone on my child=s behalf, make a claim against the
releasees described above, I will indemnify, save and hold harmless each of the releasees from any
judgment rendered against such releasee, and from any litigation expenses, including any attorney’s
fees, that may be incurred in defending against any such claim.
I certify that I have read and understood this agreement in its entirety. I understand that this document
constitutes a complete and unconditional release of all liability to the greatest extent allowed by law
and agree if any portion of this agreement is found to be invalid, the balance, notwithstanding, shall
continue in full force and effect. This agreement shall be governed by Georgia law.
