Mary Salka R.N., B.S.N., A.C.L.N.C., L.N.C.C., 120 hour mentoring course as preparation for the R.N.
for practice as a Certified Legal Nurse Consultant. This course encompassed the multi-faceted duties and
scope of the registered nurse as a legal nurse consultant. These include review of the standards of care,
medical record analysis, chart organization, medical research, and many other topics. The focus of this
program is to assist the nurse in mastering their skills in trial preparation for complex medical claims.
This includes but is not limited to medical malpractice, PI, Toxic and Federal Tort, Product Liability, and
Nursing Home litigation. This was a one to one preceptor ship working on real cases.
Professional
Experience:

7/06-present:              C. Thompson, R.N., Medical Review

         Certified Independent Nurse Consultant. President and Founder of C. Thompson, R.N., Medical
Review. Responsibilities include complete trial support to attorney-clients involved in medical-legal
cases. During my internship I assisted with cases including plaintiff medical and nursing malpractice and
nursing home neglect cases. These experiences involved intake of cases via email and or letter receiving
via post or delivery service; opening cases, reading and reviewing for initial screening; identifying all
health care providers and ensuring access to all pertinent medical records; researching key medical facts
to include conferencing with potential medical and nursing experts; and conferencing w attorney-client
regarding merit. Upon attorney clients’ decision to proceed, the records were organized and indexed to
give attorney-client the most easily understood comprehensive documentation of the existing medical
records. Critical missing records and any indication of spoliation were determined, investigated and
obtained. The complete medical record was then organized to include Bates stamp, pagination, tabulation
and final indexing. Packages were made for all experts consulted as well as a red copy for the attorney-
client to keep pristine and a black copy to be used as working copy. After a comprehensive review of case
points, reports were prepared in order to fully educate the attorney-client of pertinent anatomy,
physiology, biopathology of disease process or conditions, treatments, and appropriate standards of care.

         This is a free standing psychiatric hospital associated with Charlotte Regional Hospital.
This Hospital is comprised of three unit’s a 26 bed psychiatric unit, a 8 bed dual diagnosis
unit, and a 15 bed geriatric-psychiatric unit. Responsibilities include night shift weekend charge
nurse on the geriatric-psychiatric unit. Taking care of all geriatric patients with Alzheimer’s,
dementia and other psychiatric illnesses. Assigning staff according to their abilities, and monitoring and
making sure their duties are performed as instructed. Admitting all new patients and making sure all
admissions are correct and completed in a timely fashion and all Baker Act paper work is complete.
Occasionally passing medications if needed. Keeping doctors informed of any changes in condition of
patients.

      This is a psychiatric unit in a maximum security prison,. The psychiatric unit is comprised of a
Crisis Stabilization Unit (CSU) and a Transitional Care Unit (TCU). The CSU is for immediate
stabilization of inmate’s in psychological crisis for 1-4 weeks. The TCU is ongoing longer management
for those inmates who are stabilized but need longer and more in depth care then they can receive in
general population Also some of these inmates would not be able to manage in normal population. The
CSU is comprised of 2 quads of 26 inmates. The TCU is comprised of 2 quads of 24 inmates.
Responsibilities on the CSU include 15 minute checks on suicidal inmates. On both units routine medical
care including passing medications, admitting inmates to the ward, attending psychological emergencies
on the compound, and assessment of physical condition of inmates.

circulators, the top circulator and the bottom circulator. The top circulator was responsible for assisting
the anesthesiologist in placing all the lines. The bottom circulator was responsible for assisting the
surgeon and doing all the counts. Duties also required being on call.
