                               Training Evaluation Form




This Training Evaluation Form sets forth a template form requiring the employee to
evaluate certain training he or she recently received. The form asks several open-
ended questions to elicit employee responses and further provides a scaled chart,
allowing the employee to evaluate the program on a numerical scale. The form asks the
employee what else would be needed to improve the training and what could be revised
about the training. This document should be used by company managers to assess and
analyze existing training programs.
