					This document is used by customers to request a Return Material Authorization (“RMA”)
to allow them to return products. This form requires the customer to provide their
contact information, the order number, and the item information. Additionally, this
document can be customized to provide for any additional industry specific language
that may be necessary. This document should be used by small businesses or other
entities that sell products to consumers and want to provide consumers with a method
to return merchandise.
Return Merchandise Authorization Form
This document assists customers with returning merchandise. Some
businesses to call and receive authorization over the form before
agreeing to an RMA, while others simply allow any unsatisfied customer
to make a return as seen in this template.

Return Merchandise Authorization (RMA) Form
John’s Electronics wants only satisfied customers. If the product isn’t up to your standards -
we’ll issue a full refund of the original purchase price (minus shipping charges) via the same
payment method you used originally. We’ll process it as soon as we get it. You can help ensure a
speedy return and refund by doing the following:

1. Mark your order number on the outside of the box or shipping container

2. Complete this form and include it with your items inside the box

3. Send via the carrier of your choice to:

Important Information

      Shipping is at your expense, unless John’s electronics sent you the wrong item.

      It takes three (3) business days from the time we receive your item until the refund will
       appear on your credit card statement or online banking.

      We don’t require a separate RMA number, simply include your order number.

      If your purchase included any free promotional items (such as a DVD with purchase, free
       gift card, or complimentary batteries) please include all items in order to receive a full
       refund.
