					This a Daily Evaluation blank form. The purpose of this form is to measure the level of
understanding of training participants on daily basis. This form includes the following
questions for its participants to answer: (1) what the participant enjoyed most about that
day, (2) what participant learned during that day’s session, (3) anything participant did
not understand about the day (4) what was the most valuable thing learned that day (5)
and whether participant had any specific comments. This form contains basic questions;
however, additional questions may be added by User. Use this form to gauge
participation levels and understanding of participants in a training program.
