                     APPLICATION FOR INCLUSION OF A RESEARCH PROJECT
     This form should be used to Register Research Projects that will be supported by UCL facilities. It
     should be completed in full and returned to the UCL Data Protection Officer before Data Collection
     commences.
     If the Data has been collected from an organisation outside UCL then the Patient, or Subject, of the
     Data must have given their consent to both the collection of the Data and the transfer of the Data to
     UCL. A copy of the Patient Consent Form must be attached to this application. If you are not obtaining
     Patient Consent then an explanation must be attached.
