The following changes were included in the revised Publication 4163, dated July 8, 2004.
Page 5 – Under Request for Publication reworded 1st sentence and eliminated one step on
instructions to get to IRS.gov website.
Page 8 – Corrected the toll free number for Electronic Funds Withdrawl (Direct Debit) Deposits.
Page 11 – Added “Note” to bullet 3 and added “tentatively scheduled” to last bullet
Page 12 – Reworded 1st paragraph.
Page 14 – Removed bullets 16 and 17.
Page 15 –Under “Special Instructions for Form 8838” reworded instructions
Page 16 – Under “Foreign Address” removed Release 2 information.
Page 17 – Under “Electronic Signature Alternatives” replaced the word “Providers” with
“Originators”
Page 18 – Under “Electronic Federal Tax Payment System (EFTPS)” corrected the website name to
click on when accessing IRS.gov
Page 20 – Under “Acknowledgements” removed the “Note”.
Page 21 – Under “On-Line Filing” reworded 2nd paragraph.
Page 22 – Under “Business Name Controls” reworded 5th bullet.
Page 25 – Exhibit 1 - Under “Accepted Forms and Schedules for Forms 1120/1120S for Tax Year
2003” removed 1st sentence.
Page 28 – Exhibit 2 – replaced the “Forms and Attachments Listing” with a more current version.
Page 62 – Exhibit 5 - under “Standard Postal Service State Abbreviations and Zip Codes for US
