Scheduling a SCALE Field Trip
SCALE use is open for use to not only the students in District Five, but to any group or organization
requesting it’s use. Field trips and other activities at SCALE must be scheduled through the District
Office. This can be done by contacting Paulette Dempsey (949-2350).
Request Forms
All SCALE field trips will require a request form which should be completed no less than three weeks
before a possible trip date. The further in advance you submit the request, the more likely
transportation, if necessary, will be available. The trip must be approved by the principal. ALL
REQUESTS SHOULD BE IN WRITING - DO NOT CALL THE TRANSPORTATION DIRECTOR.
Transportation for trips are scheduled on a first come, first serve basis. Trips should not be
discussed with students until the request is approved with all signatures and returned to the school.
Transportation
When planning your field trip to SCALE, the mode of transportation should be determined by the
location of your school. Classes or organizations with field trips to SCALE from Duncan Elementary,
Freshman Academy, and Byrnes should walk, if at all possible. Walking from Beech Springs would
be recommended, especially for the older students but busses could certainly be requested. All other
schools should plan to use busses.
Bus Drivers
Remember that the teaching assistants in your school are your bus drivers. We do have a few
alternate drivers who are not teaching assistants, but they will be used on a first come first, serve
basis when requested for an approved field trip. When an alternate driver is used, you must pay the
bus driver per hour pay. Average pay is $12.00-$15.00 per hour. All drivers must be paid for after-
hour trips.
Bus Cost and Capacity
We have two types of buses - activity and school buses. Transportation cost should be based on
round trip mileage and is the responsibility of the school/students. Please note the charges and
capacity of each bus. The Transportation Director will make bus assignments when he approves the
trip.

Bus Rules
1. Food and drink will not be allowed on the bus.
2. All riders must remain in their seat while the bus is in motion.
3. Follow all other posted bus riding rules.
Permission Slips
Permission slips to SCALE should be made available to all students at the beginning of the school
year (included in initial registration information). These slips should be kept in a location that would
make it easy for teachers going to SCALE to access them. Once signed, these permission forms will
make it possible for teachers to take a field trip to SCALE, once it has been approved by the principal
and the parents/guardians notified of the trip, without obtaining individual trip permission forms.
Teachers should have a signed permission slip to attend SCALE from each student. These forms
should be with the teacher at all times in case of emergency. No child may go to SCALE without
written permission from the parent or legal guardian. Each school must use the provided SCALE
permission slip run on your school’s stationery.
SCALE Field Trip Checklist for Teachers
Calculate approximate cost of field trip, including mileage.
Complete SCALE Field Trip Request form (FTR)
Send FTR form to Principal for approval
Only when FTR form is returned to you, should you inform your students and parents of the
trip and it 's cost (At this time you will know what type bus you are using (if necessary) and will
be able to calculate exact cost).
Check to make sure that each student has a signed permission slip. If a student does not,
have one completed and returned to you prior to the field trip.
Keep permission slips with you at all times on the trip.
Upon your return, file the permission slips for later use.
