Instructions: Player Registration is $350.00 for the season. This includes uniforms (less pants). Spring
Training will be held over consecutive Saturdays, in late January or early February 9am -. until noon. These
sessions will cover skills, conditioning and strategy. It is mandatory for all players in this division. Volunteer
Buyout Required for Parents who do not sign up for one of the volunteer positions listed at the bottom of this
form. Sponsorship Families or businesses who wish to participate as a sponsor of the Pony League will be
recognized on a large banner, which will be displayed at the field for the duration of the season. Individual
Banners with the donor’s information (business or personal) will be separately displayed at the park for the
duration of the season. Proof of residency required at 1st tryout..

Dugout cleanup requires one week during the season to pickup trash and sweep dugouts, 3 days during week and
Saturday. Home team field prep requires dragging and, watering infield and chalking 1 hour prior to your child’s
home game. Field preparation requires preseason, and midseason field grooming.
Field grooming (after games) – dragging base area, homeplate and repairing pitcher’s mound.
Volunteer coordinator (2 positions) assigns volunteer positions and follows up.

Medical Release Form
TO WHOM IT MAY CONCERN:
This is to certify that I, parent or legal guardian of ___________________ a player on the
_____________ team, hereby grant permission to the adult manager, coach, and business
manager of the team to obtain medical care from any licensed physician, hospital, or medical
clinic for the player named herein at such times as either parent or legal guardian cannot be
contacted in person or by telephone. This authorization shall include all league activities,
including the period required to travel to and from those activities; and we do hereby waive,
release, absolve, indemnify and agree to hold harmless the PONY BASEBALL/SOFTBALL
organization; PONY BASEBALL, Inc.; the organizers, supervisors, participants, and persons
transporting the player to and from those activities, for any claim arising out of an injury to the
player.

A medical release form, signed by the player’s parent or legal guardian MUST
be provided for each player on the tournament team in order that physicians
and hospitals will accept players for treatment in the event of illness or injury when
the parents or legal guardian are not available.
