It must be submitted with the practice portfolio at the end of each of the modules.



This Skills Inventory is based upon the Department of Health Essence of Care Patient-

focused Benchmarks for Clinical Governance (DH 2003). In this regard, the Skills

Inventory serves two purposes, firstly the students’ practice learning is embedded within

the national benchmark framework for nursing and secondly it contributes directly to

enhancing best practice within all areas of care.



To achieve the Essence of Care Skills and to guide you in the learning, teaching and

assessment process, please refer to the Essence of Care Patient-focused Benchmarks for

Clinical Practice (DoH 2003) and your local policies and guidelines.



The Inventory is divided into two sections: Core Skills and Additional Skills.

In order to pass both modules, ALL CORE SKILLS MUST BE ACHIEVED

Shaded Core Skills must be achieved by the end of NRSDN1001.



Non-shaded Core Skills must be achieved by the end of NRSDN1002.



Even though you do not need to achieve the non-shaded skills until the end of

NRSDN1002, you may have limited opportunity in the second half of the year in which all

non-shaded skills can be achieved. You must therefore make sure that you achieve as

many non-shaded skills as possible in the first half of the year.



The Additional Skills are skills which are important but which, depending on the areas you

are placed on, you may or may not have the opportunity to achieve. Obviously, if you get

the opportunity you should practice as many skills as possible, but you will still be able to

pass the modules even if you do not get a chance to achieve some or all of the Additional

Skills.



All skills must be practiced in accordance with practice placement policies and guidelines

and performed under the supervision of a Qualified Practitioner following the standard

practice area risk assessment.



Refer to the Practice Area Code for which areas are best suited to achievement of

skills. Please ensure whilst allocated to those areas that you take the opportunity to

achieve the corresponding skills. Some skills MAY be achieved in other areas – please

ensure you take the opportunity on each placement as it arises to achieve as many skills

MENTOR AND STUDENT GUIDANCE

For each placement, three documents are to be completed;

1. Learning Agreement, 2. Practice assessment document (PAD) and 3. Practice

Skills Inventory

Each is required to confirm achievement of modular outcomes, and should be discussed

with the student at an identified time, at the beginning of each placement, midway during

the placement, and towards completion of each placement.

Any outstanding learning outcomes, practice outcomes or clinical skills from a previous

placement area should be discussed and included as part of the learning agreement for

any ensuing placement.

MENTOR RESPONSIBILITIES

The following guidance is summarised from the NMC’s Standards to Support Learning and

Assessment in Practice (NMC 2006):

Students on NMC approved pre-registration nursing education programmes must be

supported and assessed by mentors. Nurse education requires mentors to provide the full

range of support and assessment required to meet the needs of pre-registration education.

The purpose of these standards is to assure the NMC that those who make the

judgements of students have been appropriately prepared to assess performance against

the relevant NMC standards of proficiency.

Mentors who sign off all, or part of a programme leading to registration, ARE

ACCOUNTABLE TO THE NMC for their decision that the students are fit for practice and

that they have the necessary knowledge, skills and competence to take on the role of

registered nurse.

Registration provides a licence to practice and is the prime means of protecting the public.

Mentors are responsible and accountable for:

• Organising and co-ordinating student learning activities in practice

• Supervising students in learning situations and providing them with constructive

feedback on their achievements

• Setting and monitoring achievement of realistic learning objectives

• Assessing total performance- including skills, attitudes and behaviours

• Providing evidence as required by programme providers of student achievement or

lack of achievement

• Liaising with others (e.g. mentors, practice facilitators, programme leaders etc) to

provide feedback, identify any concerns about the student’s performance and agree

action as appropriate

• Providing evidence for, or acting as, sign-off mentors with regard to making

decisions about achievement of proficiency at the end of a programme.

• Check the student’s level of knowledge, skill and competence before assigning

client care activities.

• Assign patient care activities that are within the student’s capabilities and are

commensurate with their point on the course.

• Provide reasonable levels of teaching, guidance and supervision in the clinical area.

• Provide constructive feedback on the student performance at regular and indicated

points during their placement which should be documented.

• Observe/ support the students’ achievement of intended learning outcomes for a

minimum period equivalent to 40% per working week for full-time students and pro-

rata for part-time students.

Student Interviews

Initial or preliminary interview is carried out with the student within the first few days of any

placement so a learning agreement can be negotiated and formulated. An intermediate

interview should take place half way through the placement to review progress to date and

that formal feedback is given and documented. The final summative interview needs to

take place towards the end of the last week



Mentor updates

The NMC requires all qualified mentors to maintain and develop their knowledge, skills

and competence as a mentor through regular updating.

The purpose of annual updating is to ensure that mentors:

• Have current knowledge of NMC approved programmes

• Are able to discuss the implications of changes to NMC requirements

• Have an opportunity to discuss issues related to mentoring, assessment of

competence and fitness for safe and effective practice.



Mentors should be prepared to demonstrate how they have maintained and developed

their knowledge, skills and competence as a mentor.
