                                          Wednesday 28 March
   Director of IM&T Nick Thomas and Julian Evans, Foundation Trust lead, presented a paper to Joint Staff
    Negotiating Committee (staff side) on plans. Questions were asked about staff involvement and staff
    governors.




                                               Tuesday 1 May
   Director of Human Resources Christine Lloyd-Jennings came and talk to the Communications Forum (a group
    of staff who are Communications Champions from throughout the organisation) about Foundation Trust status.
    The Forum was attended by around 12 people.



                                               Monday 11 June
   Chief Executive Paul Roberts and Director of IM&T Nick Thomas gave a presentation to the Hospital Medical
    Staff Committee (HMSC) and took questions.




                                                Friday 29 June
   Chief Executive Paul Roberts, Chairman John Bull and Head of Communications Amanda Nash met with local
    MPs Gary Streeter, Linda Gilroy and Alison Seabeck on a number of issues. Foundation Trust was an item on
    the agenda and the MPs read through a draft of the consultation document.



                                                  Monday 9 July

   Chief Executive Paul Roberts attended the Joint Management Board of the Peninsula College of Medicine and
    Dentistry. Informed Board members them that consultation period would be starting on Monday 16 July and to
    look out for the document and urged them to give feedback on the plans. An electronic copy of the document
    was subsequently sent through for circulation to the board members.

                           Monday 16 July – Sunday 7 October 2007



                                                 Wednesday 18 July

   Director of Finance John Yarnold attended the public board meeting of Torbay Care Trust. He gave a
    presentation about our plans to 30 people and took questions. 40 copies of the consultation document were
    distributed. Verbal feedback was documented. See feedback sheet entitled ‘Torbay Care Trust 18 July 2007’.




                                                    Friday 20 July

   Director of Professional Practice Angela Edmunds and Non Executive Director Margaret Schwarz attended the
    Shadow Children’s Trust meeting. They gave the presentation about our plans and took questions. 15 copies
    of the document were distributed. Verbal feedback was documented. See feedback sheet entitled ‘Shadow
    Children’s Trust 20 July 2007’.


   Foundation Trust lead Julian Evans gave a presentation to the Plymouth ICT Senior Managers Forum.

   Director of IM&T Nick Thomas and Foundation Trust lead Julian Evans attended a meeting of hospital
    volunteers at which six people were present. They gave a powerpoint presentation. Verbal feedback was

   Chief Executive Paul Roberts dedicated his message to staff in the weekly news bulletin to the Foundation
    Trust application.



                                                   Tuesday 24 July

   Torbay Overview and Scrutiny Committee declined the offer of a meeting but requested a copy of the
    consultation document. An electronic copy and paper copies were sent as requested.




                                                 Wednesday 25 July

   Director of Human Resources Christine Lloyd-Jennings gave another presentation, including the powerpoint
    presentation, to 10 members of the Joint Staff Negotiating Committee. Verbal feedback was documented. See

   Foundation Trust lead Julian Evans attended the public board meeting of Devon PCT. 16 people were in
    attendance. He distributed 30 copies of the consultation document. Verbal feedback was documented. See

                                                  Thursday 26 July

   Personal letter from Chief Executive sent to all staff with their payslips. In addition, the letters were also sent to
    200-plus staff who are on maternity leave, long-term sick or those who choose to have their payslip sent to
    their home address.

   Deputy Paula Vasco-Knight and Non Executive Director Lee Paschalides visited the Royal Eye Infirmary
    where she gave a presentation to 16 staff. Verbal feedback was documented. See feedback sheet entitled ‘REI

   Chief Executive Paul Roberts attended the Board meeting of South Western Ambulance Service NHS Trust.
    He gave a presentation and handed out 20 copies of the consultation document.

   Chief Executive Paul Roberts, Foundation Trust lead Julian Evans and Head of Communications Amanda
    Nash presented the consultation document to the Trust’s meeting of 20 Clinical and Executive Directors and
    asked for feedback. Approximately 20 people were present. Verbal feedback was documented. See feedback

    electronic copy of the document in advance).

   Article in the local Plymouth newspaper, The Herald, titled: Residents to have say in running of Derriford



                                                   Monday 30 July

   News release sent to 60-plus local and regional media outlets.

   E-mail sent out to staff to remind them of staff briefing.

   BBC Spotlight highlighted on their evening programme (6.30pm) that Plymouth Hospitals NHS Trust was
    applying for Foundation Trust status.


                                                   Tuesday 31 July

   Consultation exercise in the main concourse of Derriford Hospital between 8.30am – 2.00pm. This was staffed
    by a range of Executive and Non Executive Directors and staff from the Press and Communications and
    Marketing functions. Approximately 50 consultation booklets were handed out.

   Chief Executive Paul Roberts gave a presentation to 24 members of staff at Derriford Hospital. Verbal
    feedback was documented. See feedback sheet entitled ‘Staff Briefing – 31 July 2007 PDR’.

   A member of staff informed the chief executive that they had heard about the Foundation Trust application on
    BBC Radio Cornwall

   Reporter Andy Ballantyne from local radio station Plymouth Sound interviewed Chief Executive Paul Roberts
    about the importance of becoming a Foundation Trust and why the people of Plymouth should become
    members.

   Eight consultation documents sent to the Plymouth Hospitals’ Patient and Public Involvement Forum, ahead of
    their meeting on August 14.

   E-mail sent out to staff to remind them of staff briefing.

   Chief Executive Paul Roberts and Assistant Company Secretary Gill Hunt attended a meeting of 10 staff from
    the Child and Adolescent Mental Health Service at Mount Gould. They gave a presentation and answered

   Chief Executive Paul Roberts and Assistant Company Secretary Gill Hunt attended a meeting of 24 staff from
    throughout Child Health at the Child Development Centre. They gave a presentation and answered questions.

   Medical Director Terence Lewis and Head of Communications Amanda Nash ran an open briefing for seven
    staff who wished to attend in the Plateau Conference Room. Verbal feedback was documented. See feedback

                                                Wednesday 8 August

   Letters and two copies of the consultation document sent to 50 citizens advice bureaux and 108 libraries in the
    region.

   Chief Executive Paul Roberts gave a presentation to 11 members of the Plymouth Primary Care Trust
    Professional Executive Committee. See feedback sheet entitled ‘Plym PEC 8 August 2007’.



                                                 Thursday 9 August

   Letters, posters and copies of the consultation document sent to practice managers at local GP surgeries.

   Twenty copies of the consultation document and letters to staff given to the staff library at Derriford Hospital
    after staff had made enquiries there.


                                                   Friday 10 August

   Posters and copies of the consultation document were distributed in main patient waiting areas at Derriford
    Hospital, the Royal Eye Infirmary, the Child Development Centre at Scott Business Park and the Child and
    Adolescent Mental Services at Mount Gould.

   12 copies were sent in advance to the Royal Eye Infirmary League of Friends ahead of their meeting on 12
    September which representatives from the Trust are attending.

   Chief Executive Paul Roberts ended his message to staff in the weekly news bulletin with an offer to attend
    any meetings they have and talk about the Foundation Trust application.



                                                  Monday 13 August

   Letters and copies of the consultation document sent to the following Healthcare Partners: Royal Devon and
    Exeter, Royal Cornwall, North Devon District Hospital, South Devon Healthcare NHS Trust.

   Other Partners including the Eddystone Trust, Racial Equality Council (REC), Fata He, Devon & Cornwall
    Refugee Supporrt Council, Refugee Action, Refugees First were also sent copies of letter together with copies
    of the consultation document.

   Educational institutions such as universities in Plymouth and Exeter were sent a letter and consultation
    documents.

   Elected representatives from Devon and Cornwall County Councils and the Isles of Scilly Overview and
    Scrutiny Committee were sent documents as above.

   The local Cancer Services Support Group were sent a letter and consultation document.

   The Hearing and Sight Centre and the Plymouth Primary Care Trust PPI Forum have been sent letters etc as
    above.

   Sent out copies of letter with posters and consultation document to Child Development Centre, Child and
    Adolescent Mental Health Service and Royal Eye Infirmary.



                                               Tuesday 14 August

   Chief Executive Paul Roberts and non-executive director Louise Hardy attended a meeting of the Plymouth
    Hospitals NHS Trust Public and Patient Involvement Forum. They gave a presentation to 14 people and
    answered questions. Verbal feedback was documented. See feedback sheet entitled ‘Plymouth PPI Forum 14
    August 2007’.

   Arranged for a message about the Foundation Trust application to be printed on all staff payslips at the end of
    August.
    Did you receive your letter?
    Chief Executive Paul Roberts wrote a letter to all staff in July outlining the Trust’s plans to become an NHS
    Foundation Trust. Did you receive a letter with your July payslip? If not, please contact the Press and
    Communications office on ext 53300 to obtain a copy.

   Sent copies of the consultation document and letter to Derriford Breast Support Group. Addressed to Kirsten
    Sutherland.

   Copies of the consultation document sent to Devon Overview and Scrutiny Committee.

   Emailed Plymouth Pride forum asking if they would like to arrange a meeting-waiting response.

   Sent letter and copies of the Consultation document to the Chancellor of Exeter University-Floella Benjamin.

   Sent out copies of the letter to 17 Members of Parliament in Devon and Cornwall.

   News release giving an update on the consultation process sent to 60-plus local and regional media outlets.



                                             Wednesday 15 August

   Copies of the document and letter sent to a member of the public Mr K. Pritchard, 16 Hillside Drive,
    Yealmpton, PL8 2NT who had called to request them.

   Sent personal letter to Audrey Miller, Reporter on Saltash & Cornwall District Journal together with copies of
    consultation document.

   Spoke to Kate Fenton (01404 549210), from Devon PCT PPI Forum, with regards to arranging a meeting.
    Tania (01404 549210) is responsible for the next meetings and will let us know if there is going to be a meeting
    in September. The next quarterly meeting is October 22 (too late) and the agenda for August meeting has
    already been drawn up.

                                                Thursday 16 August

   Chief Executive Paul Roberts gave a presentation to an open staff forum, which eight people attended, in the
    Conference Room at Derriford Hospital. Verbal feedback was documented. See feedback sheet entitled ‘Staff

   Chief Executive Paul Roberts gave a presentation to 30 staff at our medical records library in Bush Park.
    Verbal feedback was documented. See feedback sheet entitled ‘Bush Park 17 August 2007’.

   Chief Executive Paul Roberts and Deputy Chief Executive Paula Vasco-Knight attended a Board to Board
    meeting with Plymouth Primary Care Trust. Verbal feedback was documented on our Foundation Trust
    application. See feedback sheet entitled ‘Staff Briefing 16 August 2007 PDR’. The Primary Care Trust
    requested a box of consultation documents which were indeed supplied.

   Article in the local Plymouth newspaper, The Herald, titled: Argyle help net support for health trust.


                                                  Friday 17 August

   Senior staff manned a stand in the main entrance of Derriford Hospital talking to members of the public and
    handing out copies of the consultation document and membership forms. Some members of the public had
    seen the event advertised and turned up especially.

   Head of Communications Amanda Nash gave an interview to BBC Radio Devon regarding staff attending
    before tomorrow’s Plymouth Argyle match.

   Chief Executive Paul Roberts talked about Foundation Trust in his message to staff in the weekly new bulletin.
    Half of his message was dedicated to the topic, in particular appealing to staff to encourage their friends and
    family to join up.


                                                Saturday 18 August

   Non Executive Directors Louise Hardy and Margaret Schwarz and Head of Communications Amanda Nash
    stood outside the main gates of Plymouth Argyle FC handing out consultation booklets and Question and
    Answer sheets to interested members of the public. A photograph was taken by the Plymouth Sunday
    Independent.



                                                 Sunday 19 August

   Teletext on ITV Westcountry ran an article reporting that Plymouth Hospitals NHS Trust had consulted with
    fans on its plans to become a Foundation Trust at yesterday’s game at Home Park.




                                                 Monday 20 August

   Carolyn May from Devon Overview and Scrutiny Committee requested a further 17 copies of the consultation
    booklet - sent today.

   Sent 30 copies of the consultation booklet to Cheryl Simpson, Cornwall Overview and Scrutiny Committee.

   Took copies of the consultation booklet to Outpatients waiting areas, Chest Clinic waiting room and also main
    reception on level 6.

   Senior staff manned a stand in the main entrance of the Royal Eye Infirmary talking to members of the public
    and handing out copies of the consultation document and membership forms.



                                             Wednesday 22 August

   Sent out copies of general letter together with copy of the consultation booklet and one A4 sized poster to all
    19 support groups.

   Arranged for 4000 consultation documents and pre-paid envelopes to be sent to Quality Health Ltd, Sutton
    Manor, Palterton Lane, Sutton Scarsdale, Chesterfield, Derbyshire, S44 5UT. N.B: This equates to 40 boxes of
    the documents and a further 8 boxes of envelopes.

   Text sent to Judith Lezar for inclusion in Plymouth Community Partnership’s August Mailout. This is an
    information sheet which is mailed out to 400 local community and voluntary sector groups.

   Director of IM&T Nick Thomas attended the Directorate Managers meeting to discuss the Foundation Trust
    application with 16 senior managers. See feedback sheet entitled ‘Directorate Managers 22 August 2007’.



                                                Friday 24 August

   Chief Executive Paul Roberts gave a presentation to 16 people who attended the Plymouth Hospitals Open
    Day for PPI Forum and Overview and Scrutiny Committees. See feedback sheet entitled ‘PPI OSC Open Day
    24 August 2007’.



                                               Monday 27 August

   A mailout was undertaken with a consultation document, pre-paid return envelope and a personally addressed
    letter from the Chief Executive sent to 4,000 recent patients.




                                                Friday 31 August

   Chief Executive Paul Roberts referred once again to the Foundation Trust application in his message to staff in
    the weekly news bulletin.




                                             Tuesday 4 September

   Senior staff manned a stand in the main entrance of Derriford Hospital talking to members of the public and
    handing out copies of the consultation document and membership forms.

   Two copies of the consultation booklet were sent to Mr David Moreby, 106 Lynwood Avenue, Woodford,
    Plymouth, Devon, PL7 4SQ. Mr Moreby had visited the hospital to find out more information.

   Press release sent out to advertise the AGM and FT consultation event. Posters advertising this event also
    sent out to practice managers at local surgeries, 50 citizens advice bureaux and 108 libraries and displayed
    around Trust sites.



                                            Monday 10 September

   Article promoting the Annual General Meeting / Foundation Trust consultation in the local Plymouth
    newspaper, The Herald, titled: Public invited to view plans for hospital services.



                                            Tuesday 11 September


   Head of Communications Amanda Nash attended the public board meeting of Cornwall Primary Care Trust at
    Newquay Tennis Club. A presentation was given to the Board and members of the public attending,

   Director of HR Christine Lloyd-Jennings attended the Trustees and senior managers’ meeting of St Lukes

   Consultation documents given to Liz Pollard-Volunteer Services to give to volunteers to distribute to patients
    as they see appropriate when touring wards and patient areas.

                                            Thursday 13 September

   Head of Communications Amanda Nash attended the meeting of the League of Friends for Derriford Hospital.
    She gave a talk about Foundation Trust based around the presentation, handed out slides to aid the talk,
    circulated copies of the consultation document and answered questions.


                                             Friday 14 September

   350 copies of the consultation document were taken for delivery at Hyde Park Junior School (which covers the
    catchment areas of Mutley and Peverell) to be distributed to families of pupils.

                                            Tuesday 18 September

   Senior staff circulated in the outpatient and ward areas of the Royal Eye Infirmary talking to members of the
    public and handing out copies of the consultation document and membership forms. Some patients and
    visitors signed up as members on the day.

   Article promoting the Annual General Meeting / Foundation Trust consultation in the regional newspaper,
    Western Morning News, titled: Chance to discuss future of region’s main hospital.


                                          Wednesday 19 September

   Head of Communications Amanda Nash and Communications Officer Brydie Willis attended the Child
    Development Centre at Scott Business Park. They held a consultation for 16 children and members of their
    families, including parents and siblings. Copies of the consultation document were handed out and a 35 minute
    session was held with children and their parents, asking their views, explaining what was going to happen and

                                           Thursday 20 September

   Director of IM&T Nick Thomas attended a meeting of the Disability Action Network. He talked to 26 people
    present about the application for Foundation Trust (as a powerpoint presentation was not suitable for the
    audience). 25 hard copies of booklet has been sent in advance. See feedback sheet entitled ‘Disability Action

                                            Monday 24 September

   Chairman John Bull and Assistant Membership Secretary Gill Hunt attended a meeting of the South West
    Peninsula Deanery. They gave a presentation to approximately 18 people and took questions.


                                            Tuesday 25 September

   Chairman John Bull and Julian Evans attended a meeting of Cornwall Overview and Scrutiny Committee. Sixty
    people attended. An electronic version and 30 hard copies of the document were sent over in advance and
    distributed with the papers. A discussion took place. See feedback sheet entitled ‘Cornwall OSC 25 September
    2007’.

                                          Thursday 27 September

   Non-executive director and vice-chairman of Plymouth Hospitals NHS Trust, Louise Hardy, was interviewed
    live on the BBC Radio Devon, Plymouth breakfast programme about the annual general meeting and the
    Foundation Trust consultation.



                                           Friday 28 September

   A presentation and discussion about plans to become a Foundation Trust were part of the open Annual
    General Meeting. This was given by Chief Executive Paul Roberts. Approximately 10 members of the public
    and 30 staff attended. See feedback sheet entitled ‘AGM 28 September 2007’.

   An article appeared in the local Plymouth newspaper the Herald about the AGM and Foundation Trust event,
    inviting people to come along.


                                            Thursday 4 October

   Director of Professional Practice Angela Edmunds attended a meeting of the Patient and Public Involvement
    group for Royal Cornwall Hospitals NHS Trust. The group had asked for a presentation on Plymouth’s plans.


                                             Friday 5 October

   MP Alison Seabeck sent a press release out appealing to people across Devon and Cornwall to become
    members. Coverage was given to this in the local newspaper, the Herald the following week.
