RE: Credit Report #123456
Dear Customer Service Rep: This letter constitutes FORMAL NOTICE that you are hereby instructed to place a SECURITY FREEZE and NOT TO RELEASE any of my private information, including my name, address, birthdate, telephone number, SSN, employment or credit history to any person or agency without my express prior written approval, in accordance with Civil Code Section 1785.11.2, and as published in Witkin:
A consumer may elect to place a security freeze on his or her credit report by making a request in writing to a consumer credit reporting agency (CRA). "Security freeze" means a notice placed in a consumer's credit report, at the request of the consumer and subject to certain exceptions, that prohibits the CRA from releasing the consumer's credit report or any information from it without the express authorization of the consumer. Items included in this freeze include: name, address, birthdate, SSN and credit info. A freeze may always be lifted temporarily for the extension of credit and a unique personal identification number or password will be provided to the consumer for that purpose…
This is also NOTICE that Compass Bank Visa account 1234 was voluntarily closed by me on April 9, 2003 when payment was made in full, as evidenced by Certified Mail RR #________. They are no longer to receive access to any of my private information. Thank you for your prompt attention to this notice. I look forward to receiving the error resolution sought in my April 10, 2003 notice to you and proof of correction within the 30 days specified by FCRA.
