 This page is for describing general tree form and other health factors. The tree tag numbers
 should be updated when you enter the numbers on the Source-Info sheet. Use the drop down
 boxes to indicate the form that characterizes that tree. In the lower part of the page, check the
 appropriate boxes for the other factors that are noted on each tree. At the bottom, there are
 questions that may be answered in the text box below each question.
