Faculty Commitment to Advise (only for Rehabilitation Sciences track applicants)
Applicants who are accepted into the PhD in Kinesiology, Nutrition and Rehabilitation Sciences (KNR) –
Rehabilitation Sciences track will be matched with one of the Rehabilitation faculty members who is a
member of the Graduate College. This faculty member will act as the student’s academic advisor and
mentor throughout the PhD program.
The program requires that you submit your application with a dedicated advisor. If you have spoken with a
faculty member who has committed to advising you if you are accepted into the KNR PhD program,
please have that individual sign below. If obtaining the faculty member’s signature is complicated by
geographical separation or other practical issues, it will be acceptable for you to attach an e-mail from the
faculty member indicating their willingness to advise you if you are accepted into the KNR PhD
