ANNEX B: PROJECT MONITORING AND EVALUATION PLAN
Purpose
To help project applicants develop a Monitoring and Evaluation roadmap for the proposed grant activities. When properly completed, reviewed, and approved, this BACP Project M&E Plan serves for
project monitoring and evaluation to ensure i) quality and effectiveness of project implementation, ii) progress against targets, iii) achievement of project goals and objectives, and iv) consistency in
delivery.

Instructions
Refer back to Question 1 under Section I: Project Overview in Annex A to complete the M&E Plan. List all of the objectives by number and their related activities exactly as you identified them in
Question 1. For all activities and their corresponding objectives that you listed in Question 1, fill in the cells in the BACP Project M&E Plan with the following items. Use the example in Worksheet 3
for additional guidance.
Project Objective Number - Refer back to Section I, Question 1 of the Application (Annex A), where you defined your project objectives and activities. In the spreadsheet, please fill in the number of
the Objective that is associated with each activitiy that you will identify in column 2, Project Activity.
Project Activity - Refer back to Section I, Question 1 of the Application (Annex A), where you defined your project objectives and activities and copy from this section each of the activities listed.
Please note that only one activity should be reference per row. You do not need to copy into this spreadsheet the contextual information provided for each activity in Section I. If activities are too
cumbersome (ie. too many discrete activities discribed together), please refine your activity list in Section I. and disaggregate as necessary. Clear, concise, discrete activities will facilitate a more
managable M&E system.
Start date / End date - The start and end dates should represent the period of performance for each discrete activity. Start dates should not occur prior to grant signing and end dates should reflect
the completion of the output/outcome associated with that activity. See below for information related to Outputs and Outcomes.
Output – This refers to a product or service that results directly from a project activity, e.g., training events held, reports produced, strategic plans developed, studies completed, publications
produced. The outputs listed here should be direct results of the project activities stated in your Grant Application Form. Some examples of outputs for each BACP Component can be found on
Worksheet 2 of this Excel file.
Outcome – This refers to a change, benefit, learning or other effect that happen as a result of the use of an output, e.g. change in knowledge, skills, behavior, productivity or performance. Some
examples of outcomes for each BACP Component can be found on Worksheet 2 of this Excel file.

Indicator – This refers to a single, quantitative or qualitative measure that is used to demonstrate change and details the extent to which project results (i.e., outputs and outcomes) are being
achieved. The indicators listed here should be in accordance with your Grant Application Form. Some examples of output and outcome indicators for each BACP Component can be found on
Worksheet 2 of this Excel file.
Unit of Measurement – This refers to the unit which is used to measure the indicator (e.g., number, %, hectare, tons, etc.)
Means of Verification – This refers to the proposed source or method used to gather information on the state of the proposed indicator.
Baseline – This refers to the condition or performance of an indicator prior to treatment or project intervention. Include here the value of the indicator before the start of the project.
Target – Include here the desired goal to be reached at the end of the project.
Responsible person – Include here the name of the person responsible for collecting the information necessary for monitoring this particular indicator.
Data collection frequency – Indicate here how often the information will be collected.

Critial Assumptions - This refers to a general condition under which the development hypothesis or strategy for achieving an outcome will hold true. Critical assumptions are outside the control or
influence of BACP and its partners, but they reflect conditions likely to affect the achievement of outcomes in the Results Framework, e.g. the level of world prices, or the openness of export markets.
Critical assumptions can be determined by asking “What conditions should exist in addition to my objectives in order to achieve the outcome?”. They are factors that grantees should anticipate, try to
influence, and develop contingency plans for in case things go wrong.
BACP Component – Indicate here to which BACP component (1 to 4) the indicator will contribute.

The tables below provide examples of key outputs, outcomes, and performance indicators that applicants could use to monitor
and evaluate their project activities. The following questions may serve as additional guidance for developing a project-specific
Monitoring and Evaluation Plan:
- Establishing the purpose and scope – Why do we need M&E and how comprehensive should the M&E system be?
- Identifying performance questions, information needs and indicators – What do we need to know for monitoring and
evaluating the project in order to manage it well?
- Planning data collection and organization – How will the required information be gathered and organized?
- Planning data analysis, critical reflection processes and events – How will we make sense of the information gathered and
