This form and applicable deposits must be received in the ISV USA office no later than 105 days prior to departure (see address below).
Your airfare will be included on your Primary Invoice, which will be mailed 80 days prior to your departure date. All payments must be
received in the ISV USA office no later than 65 days prior to departure. If your full airfare payment is not received on or before 65 days
prior to departure, your reserved space on the ISV Group Flight will be released. Exact deadline dates can be found in the ISV
Handbook.

I would like to reserve my seat on the Thai Airways ISV Group Flight departing from Los Angeles, California (LAX).
Flight costs are subject to change due to fluctuating fuel surcharges, taxes, etc. implemented by the airlines. Prices may increase by as
much as, but not more than $100.00 USD.
Enclosed is my $100.00 USD non-refundable, non-transferable deposit.

THAILAND OPTIONAL ACTIVITY PACKAGE (OAP)
I would like to participate in the Thailand OAP for an additional cost of $185.00 USD.
Cost will be added to your invoice. See OAP information at www.isvonline.org/thoapusa.php.
No deposit is required.
SPECIAL FLIGHT REQUEST
To request a flight extension or make any deviations to the ISV Group Flight, clearly state your request in an email to
info@isvonline.org with “SPECIAL FLIGHT REQUEST” in the subject line. An ISV staff member will reply with additional
information.
