Title: Designation of Privacy Officer

Policy #: PV- 11

Purpose: To provide a clear definition of the responsibilities of the Beth Israel Deaconess
Medical Center BIDMC Organized Health Care Arrangement (OHCA) as defined by policy
PV-01 Beth Israel Deaconess Medical Center Organized Health Care Arrangement. Privacy
Officer(s) is defined under the Standards for Privacy of Individually Identifiable Health
Information (Privacy Rule) and Health Insurance Portability and Accountability Act of 1996
(HIPAA).


Policy Statement - Beth Israel Deaconess Medical Center is committed to ensuring the
privacy and security of patient health information (PHI) as defined by policy PV-04
Confidentiality. In order to manage the facilitation and implementation of activities related to
the privacy and security of PHI, each entity under the BIDMC OHCA will appoint and maintain
an internal Privacy Officer position. The Privacy Officer will be trained on all policies and
procedures necessary to fulfill his or her responsibilities in ensuring the privacy of protected
health information.

1) Each entity under the BIDMC OHCA will designate a Privacy Officer responsible for
   oversight of the policies and procedures regarding the privacy of health information.

2) The Privacy Officer will be responsible for overseeing, in conjunction with the Chief
   Information Officer, all policies and procedures for the secure transmission and storage of
   PHI including:
   a) Secure transmission and storage of PHI in any form;
   b) Controlling access to PHI;
   c) The secure management of PHI;
   d) The proper use and disclosure of PHI at the request of the individual;
   e) The proper use and disclosure of PHI without the authorization of the individual;
   f) Initiating authorization for the use or disclosure of PHI;
   g) Individual rights regarding PHI;
   h) Developing and maintaining contracts with business associates regarding the use and
       disclosure of PHI;
   i) The proper use of the BIDMC Notice of Privacy Practices;
   j) Auditing access to PHI;
   k) The maintenance of records regarding access to PHI.

3) In conjunction with the BIDMC Human Resources department, the Privacy Officer will be
    responsible for overseeing the annual training of all Privacy policies and procedures and
    disciplinary procedures related to the breach of PHI by an employee.
