Students must apply ONE YEAR in ADVANCE to receive a Student Teaching Placement.

Before you student teach, you must have completed the following requirements: a. FULLY admitted to UNI-TE; b. certified in High Risk Behaviors/Substance
Abuse; c. cum GPA 2.5+ overall; d. UNI GPA 2.5+; e. department/major GPA 2.5+; f. Professional Education Sequence courses 2.5+ GPA and C- or above in
PES courses; g. ALL METHODS courses C or higher; and successful completion of Level II, acceptable professional dispositions, and availability of placement.

               Declare a major.
                
               Complete the Declaration of Curriculum form and send it to: UNI 2+2 Advisor, SEC 618, Schindler Education Center @ UNI, Cedar Falls, Iowa
               50614-0606.
             You may request a Declaration of Curriculum form by contacting the UNI Registrar’s Office or downloading it at http://www.uni.edu/advising/faq/
             You should include your selection of either a 24-hour area of concentration in the “minor” section (social studies usually has the most “double-count”
               courses) or the second major being offered: Middle Level Education.
             The D of C form will ensure that you are identified as a 2+2 student and receive assistance specifically designed to support you as a distance-education
               student.

                   An acceptably completed and submitted application will ensure you have access to the UNI online monitoring system for teacher education majors, the
                    UNITED System.
                   View your progress via the UNI online system by going to www.uni.edu and entering “MyUNIverse” and then going to the “UNI-T-ED” System.
                              As soon as possible complete all requirements for Admission to UNI Teacher Education
                                                                         …no later than AUGUST 1, 2011
                   Evidence of completion of ALL REQUIREMENTS FOR ADMISSION TO TEACHER EDUCATION (such as COURSEWORK and PRAXIS I EXAM
                    SCORES) must be sent ASAP to UNI to be recorded.
                   At the beginning of each semester (week 4) and at the end of each semester (as soon as grades have been posted) REQUEST COPIES OF YOUR CC
                    TRANSCRIPTS BE SENT directly to the UNI REGISTRAR’S OFFICE, Gilchrist Hall, Cedar Falls, Iowa 50614-0006.
                   You must have met all requirements by AUGUST 1, 2011 to attend the first day of classes in the Fall 2011 courses.

                                                                       STUDENT REQUEST FORMS
                   Special requests such as course-substitution OR waiver of a course or program requirement, must be made using a “Student Request Form.”

                   All SRFs should be sent to the 2+2 Academic Advisor for an initial recommendation decision.

                   Note: The most common SRF submission from 2+2 students is for the 200:017Field Experience to be waived. Those 2+2 students who are
                    currently or have been recently employed in an Iowa DE-approved PK-Grade 12 school setting AND who need credit for 200:017 may
                    apply for a waiver of 200:017. These students must secure a letter from his/her supervisor on business letterhead (with email and phone contact
                    information) detailing the length of employment, age level of students, duties engaged in, and quality of the work performed. The SRF and supporting
                    documentation should be sent to the 2+2 Academic Advisor for initial recommendation. If approved, the Advisor will send the request to the appropriate
                    parties for their decision. You should check your records on MyUNIverse for evidence of course approval. Contact the 2+2 Academic Advisor or the
