Water Sports – Declaration
1. I declare that all the information given above is true and correct. All participants have obtained the consent of
their parents/guardians or the persons authorised by their parents/guardians to participate in the above activity.
They suffer no illness which renders them unsuitable to participate in the activity. The organiser(s) shall not be
liable for the death or any injury of the participant caused during the activity due to his/her own negligence, poor
health or physical inability.

1. Schools may submit applications for continuous training for an academic termall at once.
2. Please delete as appropriate.
3. Please fix the training dates and time according to the specified requirement in the prospectus. The Hong
Kong, China Rowing Association (HKCRA) will assign coaches to oversee the courses on the specified
dates subject to the actual conditions and the agreement with the schools concerned.
4. For the Star 2 & Star 3 and Continuous Training Courses of Water Rowing, students are required to
arrange their own transportation to and from the Rowing Centre of the HKCRA at Sha Tin.
5. For “Indoor Rowing Outreach Coaching Programme”, the participating school should sign the designated
undertaking and fax the form to the School Sport Programme and The Hong Kong, China Rowing
Association (HKCRA) (fax no.: 26014477) before the start of the courses, in order to confirm that the
schools are fully understood the arrangement of the course.
Remarks: 1. A separate form is required for each course. In case of oversubscription, enrolment shall be determined
2. The duly completed enrolment form together with a cheque for the payment of the enrollment fee
(payable to the “Hong Kong, China Rowing Association” and with the school name marked clearly on
the back) should be sent by post to the School Sports Programme Unit, 1/F, Leisure and Cultural Services
Headquarters, 1-3 Pai Tau Street, Sha Tin.

1 【Course Arrangements】
We understand and consent that we will conduct the School Sports Programme － Indoor Rowing
Outreach Coaching Programme by arranging coach(es) on our own. We shall arrange on our own
qualified indoor rowing coach(es) recognised by the Hong Kong, China Rowing Association (HKCRA)
to supervise our students in training and to devise training programmes and intensity for individual
trainees according to their physical fitness condition. And the training is conducted under supervision
and in a safe manner. During the course, the HKCRA will assign an instructor to visit the school twice
(4 hours in total) to provide technical advice and assess the progress of the course. In case we fail to
conduct the course in accordance with the HKCRA course instructions or properly keep or use the indoor
rowers borrowed from the HKCRA, the HKCRA shall be entitled to terminate the course and take over
the equipment borrowed. The course fees will not be refunded.
2 【Provision of Qualified Coaches】
We will comply with the relevant requirements and arrange qualified coach(es) as stated above to
supervise the training.
3 【Proper Use of Equipment】
We understand that we shall properly use and keep the two indoor rowers borrowed from the HKCRA.
The equipment shall only be used in a safe environment under the supervision of qualified person(s).
To avoid accidents, we shall exercise due diligence in safekeeping and checking the said equipment. In
case of any negligence which leads to any damage to the equipment, we shall report the incident and be
liable to pay the compensation so incurred. We also understand that the equipment will be borrowed for
not more than 6 months and the organiser shall be entitled to resume the equipment upon completion of
4 【Participant’s Physical Condition】
We understand that every participant has obtained the consent of his/her parent/guardian or any person
authorised by his/her parent/guardian for taking part in the above activity and declare that he/she is not
suffering from any illness that may render him/her not suitable for participating in this activity. The
Leisure and Cultural Services Department and the organiser shall not be held liable for the death or injury
of the participant caused by his/her own negligence, poor health or physical inability while participating
in this activity. Moreover, we shall pay attention to his/her health condition on the training days to
ensure that he/she is fit enough to take part in the training.
We understand that we have the responsibility to ensure that our students will take part in the activity in a
safe manner and under the supervision of qualified person(s).
