                      Those local exchange carriers with a bona fide retail request program (66 §3014 (c) ) must
                      provide semiannual reports (66 §3014 (c)(9) ) to the commission and the Department of
                      Community and Economic Development on the number of requests for advanced services
                      received during the reporting period by exchange or density cell and the action taken on
                      requests meeting the requirements of this subsection.

                     All eligible telecommunications carriers (ETCs) must advertise the availability of Lifeline Service FCC Guidelines for

Annually   Federal   UNIVERSAL SERVICE ADVERTISING OF BASIC SERVICE
                     Companies must advertise, ANNUALLY, the availability of basic, supported services throughout Sample Bill Insert
                     the company service area. A bill insert is not sufficient.

                     The FCC developed an electronic system for reporting telecommunications service interruptions.          Outage Reporting
                     Outage reporting is mandatory for wireline, wireless, paging, cable, satellite, SS7, or 911 service
                     providers. Also available at the link to the right is a user manual to familiarize yourself with the
                     system.

                     188A1.doc. Please familiarize yourself with what is considered “reportable” under these rules
                     (Wireline information begins on Page 47 of this order).

                    All telecommunications common carriers that expect to contribute more than $10,000 to federal Universal Service Fund Form and Instructions
                    (USF) support mechanisms must file this quarterly form.
                    DeMinimus Exemption: Only companies that fall under the DeMinimus Exemption would be exempt from this form. Go

    1-Feb Federal   FORM 502: NUMBERING RESOURCE UTILIZATION/FORECAST REPORT

                   Only due for those companies that did NOT request a waiver of this filing. Each year, PTA suggests that each
                   company request a waiver of the March filing (when making the filing in Aug.). If you received your approval for a
                   waiver, you only need to file in August.
    31-Mar State   TELECOMMUNICATIONS RELAY TRACKING
                   All Incumbent and Competitive Local Exchange Carriers (ILECs and CLECs) are required by 52 Pa. Code §
                   63.37, to file the TRS Annual Tracking Report. It summarizes the TRS surcharge revenue that each carrier
                   collects from their customers.

                   On-line reporting of residential account information as listed in 64.201(c) of PA Code.                         Online Reporting
                   Companies will need a User ID and password (same as last year). If you can't locate your User ID or need to re-
                   establish your password, call Donna. All ILECs and CLECs need to file (unless the CLEC doesn't offer
                   residential service)
                   PROPRIETARY INFORMATION: Since each company has its own User ID and password, no other company
                   can view another company's report. In addition, there is a comment area at the bottom of the 64.201 form where
                   you can indicate proprietary status if you choose.

    31-Mar State     PUC ASSESSMENT REPORT (GAO)
                     Every public utility shall file with the PUC a statement under oath showing its gross intrastate operating revenues
                     for the preceding calendar year. Reference: 66 §510 (b) - (ILECs, CLECs, IXCs and CAPs)
                          In order to ensure that member companies complete the PUC Assessment Report in an accurate,                    Report and

                     LINE 1 OF THE REPORT SHOULD:
                      - Reflect gross operating revenues for which tariffs are on file with the PUC.
                      - Include gross ITORP access revenues - ITORP expenses should not be netted against ITORP revenues
                      - Include uncollectibles, which means uncollectibles should be netted against the corresponding revenues
                     reported in line 1.
                      - NOT include non-tariff services such as billing and collection, rent, directory advertising, etc

                      - Any interstate revenues which have been included on line 1. For most, if not all PTA companies, line 2 will
                     likely be $0.
                     LINE 3 OF THE REPORT SHOULD INCLUDE:
                      - The mathematical results from subtracting line 2 from line 1.

    31-Mar State     PA UNIVERSAL SERVICE FUND REPORTING
                     Data collection system for PA USF: Data collected determines the size of the fund for the following year and
                     aids in setting the assessment rate.
                     Each year carriers must submit Intrastate Retail Revenues to Solix, Inc. On 1-8-10, Solix mailed packets to
                     members with UserName and Password information for access to the Data Collection website, which opened on
                     1-15-10 and will close on 3-31-10.

                     Every carrier must prepare, execute and file annually, a certificate indicating it is operating in compliance with the
                     FCC’s CPNI rules, including information on any actions taken against data brokers, and any CPNI-related                   47 C.F.R.

     1-Mar Federal   FCC FORM 477: LOCAL COMPETITION & BROADBAND REPORTING
                 FCC Form 477 collects information about broadband connections to end user locations, wired and wireless local         Online Reporting
                 telephone services, and interconnected VoIP services, in individual states. Data obtained from this form will be
                 used to describe the deployment of broadband infrastructure and competition to provide local
                 telecommunications services. NEW: This form is now available to file online.
                 Those responsible for filing this form include:
                 1) Facilities-based Providers of Broadband Connections to End User Locations
                 2) Providers of Wired or Fixed Wireless Local Exchange Telephone Service,
                 3) Providers of Interconnected Voice over Internet Protocol (VoIP) Service
                 4) Providers of Mobile Telephony Service
                    Confidential treatment of the data submitted by your company may be requested by indicating "non-
                                                      disclosure" on page one of the form.
30-Mar Federal   FCC FORM 507: ACTUAL LINE COUNT DATA
                 Line count updates are required to recalculate a carrier's per line universal service support, and is filed with
                 USAC. Only carriers WITH competitive entry must file this report on a quarterly basis - otherwise this form is
                 only due July 31. (see July for more details)                                                                            Form 507
                 NECA will prepare this data on behalf of rate-of-return carriers in the common line pool, but will require your        Contact NECA

31-Mar Federal   FCC FORM 492: RATE OF RETURN REPORT (Preliminary)
                 The preliminary report shall be filed in triplicate with the FCC within three months after the end of each calendar      Form 492
                 year by each local exchange carrier or group of affiliated carriers which is not subject to Sections 61.41 through
                 61.49 of the Commission’s Rules and which has filed individual access tariffs during the enforcement period.

                 NECA files this report for those companies that participate in both the Common Line and TS pools.

31-Mar Federal   FCC FORM 508: PROJECTED ANNUAL COMMON LINE REVENUE REQUIREMENT
                 Pursuant to Section 54.903(a)(1) of the Federal Communications Commission’s rules each rate-of-return                Form 508
                 incumbent local exchange carrier (ILEC) that operate in study areas of rate-of-return carriers must provide
                 information needed to calculate the Projected Annual Common Line Revenue Requirement for each of its study
                 areas in the upcoming funding year. In addition, ILECs must provide line count data by class of service to allow
                 USAC to project end-user revenue amounts for the ICLS calculation.
                     NECA will prepare this data on behalf of rate-of-return carriers in the common line pool. NECA will          Contact NECA at

31-Mar Federal   FCC FORM 525: Competitive Carrier Line Count Quarterly Report
Competitive eligible telecommunications carriers (CETCs) are eligible to receive high cost support if they serve   Form 525
lines in an incumbent carrier's service area, and that incumbent carrier receives high cost support. CETCs are
eligible to receive the same per-line support amount received by the incumbent carrier in whose study area the
CETC serves lines. Unlike the incumbent carriers, CETCs will use FCC Form 525 to submit their line count data
to USAC.

                   companies may use their own language as long as they get it approved by BCS.

                   The report summarizes a corporation's financial and accounting data. (ILECs, CLECs, IXCs, & CAPs)

                   Utilities under the reporting requirements of 52 Pa. Code §§27.10, 61.28, 63.36, 6 5.19, 59.48 and 57.47                Form

   1-Apr Federal   ARMIS Reports
                     No ARMIS reports are required of the approximately 1200 small companies with annual revenues
                                                 below the current revenue threshold of $125M.
                  The Automated Reporting Management Information System (ARMIS) was initiated in 1987 for collecting              Online ARMIS
                  financial and operational data from the largest carriers. Additional ARMIS reports were added in 1991 to          reporting
                  collect service quality and network infrastructure information from local exchange carriers subject to price
                  cap regulations, in 1992 for the collection of statistical data formerly included in Form M, and in 1995 for
                  monitoring video dial tone investment, expense and revenue data (the video dial tone reporting requirement
                  was eliminated by the Telecommunications Act of 1996.) ARMIS filing requirements were reduced
                  significantly for 2008 data by Commission forbearance orders.

  1-Apr Federal   FCC FORM 499-A: TELECOMMUNICATIONS REPORTING WORKSHEETS
                  This form must be filed by all contributors to the Universal Service Fund (USF) support mechanisms, the This form can be
                  Telecommunications Relay Service (TRS) Fund, the cost recovery mechanism for North American submitted online.
                  Numbering Plan Administration (NANPA), and the shared costs of local number portability (LNP).
                  Contributors include every telecommunications carrier that provides interstate, intrastate, and international
                  telecommunications, and certain other entities that provide interstate telecommunications for a fee.

 30-Jun Federal   CERTIFICATION OF INTERSTATE COMMON LINE SUPPORT
                  Section 54.904 of FCC rules requires that any rate of return carrier and CETCs desiring to receive Interstate         sample certification
                  Common Line Support (ICLS) and Long Term Support (LTS) beginning July 1st, must file a certification with                   letter
                  both the administrator of ICLS (the Universal Service Administrative Company – USAC) and with the FCC, that
                  the support will be used for its intended purpose. In other words, carriers are required to certify that their ICLS
                  and LTS support is being used consistent with Section 254(e) of the Communications Act.

                   This form is used to obtain reimbursement of low-income Universal Service Fund support claims.
                   This form must be completed monthly and filed quarterly to obtain reimbursement of low-income Universal            Filing Instructions
                   Service Fund support claims. Completed worksheet and certification should be returned to USAC as shown
                   below. You should submit three separate worksheets per quarter, i.e., one worksheet for each month within the
                   quarter. NEW: This form can now be filed Online (preferred method)
  31-Jul Federal   FCC FORM 507: ACTUAL LINE COUNT DATA
                   Pursuant to Section 54.903(a)(1) of the Federal Communications Commission’s rules, each rate-of-return                 Form 507
                   incumbent local exchange carrier (ILEC) and competitive eligible telecommunications carrier (CETC) that
                   operate in study areas of rate-of-return carrier must provide line count information necessary to compute
                   Interstate Common Line Support to USAC, except for the line information required to comply with 54.903(b)
                   described under the revenue requirement section in FCC Form 508.
                   NECA will prepare this data on behalf of rate-of-return carriers in the common line pool, but will require your     Contact NECA

                   RURAL CARRIER CERTIFICATION (should only be filed if company status changes since its original filing)
                   Companies under 100,000 Access Lines: The need for companies with less than 100,000 access lines to
                   annually re-certify as rural carriers has been eliminated. The FCC ruling affects carriers that have previously
                   filed at the study area level for self-certification as rural carriers. These carriers need to file only if their status
                   changes since their original certification was filed in 1998.
                   Companies over 100,000 Access Lines: Companies with access line count over 100,000 should have filed
                   self-certification letters by July 1, 2000 to establish their year 2001 status. In 2001 and thereafter, a carrier
                   serving areas with more than 100,000 access lines, and claiming rural status, will be required to file ONLY if its
                   status changes. A carrier serving more than 100,000 access lines that files for rural status on or after 7/1/00,
                   must submit self-certification information to the FCC.
  31-Jul Federal   FCC FORM 525: Competitive Carrier Line Count Quarterly Report
                   Competitive eligible telecommunications carriers (CETCs) are eligible to receive high cost support if they serve   Form 525
                   lines in an incumbent carrier's service area, and that incumbent carrier receives high cost support. CETCs are
                   eligible to receive the same per-line support amount received by the incumbent carrier in whose study area the
                   CETC serves lines. Unlike the incumbent carriers, CETCs will use FCC Form 525 to submit their line count data
                   to USAC.
  31-Jul Federal   SEMI-ANNUAL ENTITY ACCESS/USAGE REPORT
                   To assist in the administration of Carrier Identification Codes (CICs), access providers, entities, and Billing & ATIS-0300050 CIC
                   Collections Clearinghouses are requested to provide CIC access and usage information to North American               Assignment
                   Numbering Plan Administration (NANPA). The access/usage reporting requirements described below have been Guidelines (Pages
                   developed for use by the access providers, entities with CIC assignments, and Billing & Collections                    28-38))
                   Clearinghouses in preparing the reports for 4-digit Feature Group B (FGB) and Feature Group D (FGD) CICs.

20-Aug State      STATE TAX ADJUSTMENT SURCHARGE
                  In compliance with Title 52, §69.52.
                  The effective date is September 1st; therefore, the filing needs to be made at least 10 days prior to that date.

                  PTA's Management Accounting Committee will prepare the Excel file for members to use in figuring their
                  calculations. Also needed (as supporting data) are: 1) PURTA Notice of Determination, which will come from DOR
                  and dated August of current year. 2) Settlement Notice - also from DOR but could be several years old. The
                  Committee also suggests that companies file a waiver with this filing for the March STAS filing of next year.

                  Reminder: As stated in PTA Member Bulletin 09-43, due to the postponement of the scheduled phase-out of the              PTA Member
                  Capital Stock and Franchise Tax rate in 2009, companies should make an adjustment to their STAS. Companies               Bulletin 09-43
                  were given the option to do it in November of 2009 or wait until the August STAS filing. If you did not make the
                  adjustment in November, you must do so in August 2010.

                  PTA will send members an affidavit that must be signed by an officer of the company and notarized. The affidavit
                  certifies that the carrier will use any high cost support received for the purpose it was intended. The affidavit gets
                  sent back to PTA. PTA will file the affidavits with the PUC for their October 1st deadline.

 1-Aug Federal    FORM 502: NUMBERING RESOURCE UTILIZATION/FORECAST REPORT
                  Any wireless or wireline carrier (including paging companies) that have received number blocks-including 100, 1,000, Online Form
                  or 10,000 number blocks (from NANP, a Pooling Administrator, or another carrier) must file Form 502.
31-Aug Federal   LIFELINE CERTIFICATION and VERIFICATION
                 In August 2008, USAC revised the sample letter for these filings and merged them into one document - taking care            sample letter
                 of both requirements with one letter. PTA members should check box 2 and fill in columns A-E.
                 CERTIFICATION: Eligible Telecommunications Carriers must submit this certification annually to USAC - an officer
                 of an ETC must certify that the company is in compliance with state Lifeline and Link Up income certification and
                 verification procedures and that, to the best of his or her knowledge, documentation of income was provided.

                 VERIFICATION: Eligible Telecommunications Carriers (ETCs) are required to verify the continued eligibility of a           Statistically Valid

  1-Sep Federal   FCC FORM 477: LOCAL COMPETITION & BROADBAND REPORTING
                  FCC Form 477 collects information about broadband connections to end user locations, wired and wireless local           Online Reporting
                  telephone services, and interconnected VoIP services, in individual states. Data obtained from this form will be
                  used to describe the deployment of broadband infrastructure and competition to provide local
                  telecommunications services. NEW: This form is now available to file online.
                  Those responsible for filing this form include:
                  1) Facilities-based Providers of Broadband Connections to End User Locations
                  2) Providers of Wired or Fixed Wireless Local Exchange Telephone Service,
                  3) Providers of Interconnected Voice over Internet Protocol (VoIP) Service
                  4) Providers of Mobile Telephony Service
                     Confidential treatment of the data submitted by your company may be requested by indicating "non-
                                                       disclosure" on page one of the form.
 30-Sep Federal   FCC FORM 492: RATE OF RETURN REPORT (FINAL) - makes corrections
                  The preliminary report should have been filed in March with the FCC by each local exchange carrier or group of             Form 492
                  affiliated carriers which is not subject to Sections 61.41 through 61.49 of the Commission’s Rules and which has
                  filed individual access tariffs during the enforcement period.
                  NECA files this report for those companies that participate in both the Common Line and TS pools.

                   The Pennsylvania Public Utility Commission must submit a letter to the FCC and USAC certifying rural carriers in
                   Pennsylvania will use any high cost support received only for the provision, maintenance, and upgrading of the
                   facilities and services eligible for support.
                   Members will receive an affidavit from PTA in August that needs notarized and returned. This will be given to the
                   PUC for its filing.
   1-Oct Federal   LOCAL SWITCHING SUPPORT (NEXT YEAR'S PROJECTIONS)
                   Each incumbent local exchange carrier (cost and average schedule companies) that is not a member of the                       Forms
                   NECA Traffic Sensitive tariff, that has been designated an eligible telecommunications carrier, and that serves a
                   study area with 50,000 or fewer access lines shall, for each study area, provide the Administrator with the
                   projected total unseparated dollar amount assigned to each account in 47 C.F.R. 54.301(b). The data collected
                   will be used by USAC to calculate each carrier’s local switching support, consistent with section 54.301 of the
                   commission’s rules. Average scheduled companies must submit form LSSa and cost companies must submit
                   form LSSc.
                   NECA will be preparing this data on behalf of rate-of-return carriers in the traffic sensitive pool, but will require     contact NECA
                   your authorization to submit to USAC. To do so, contact your NECA representative or use the website at
                   www.necainfo.org.

                  Common Line Support that each carrier receives for each of its study areas, based on the carrier’s common
                  line actual costs in the preceding calendar year. This information must be submitted on December 31st of each
                  year. This data in the form will be used to calculate and adjust the amount of support, if any, that each
                  reporting carrier is eligible to receive from the Interstate Common Line Support Mechanism.
