Use the table below to list desired titles and authors. Include the price per book (use
book list as reference) and number of copies ordered. Calculate the cost for each line.
Total the last 2 columns in the gray area at the bottom of the table.
Make your check payable to the PSPA.
Take the completed order form and your check to the Library.
