“Everything You Wanted to Know About Course Action
Forms, Degree Requests, and Degree Sheets”
Target Audience: All individuals in colleges and academic
departments who prepare or approve curricular requests.
The workshop will include interactive discussions regarding the
course action form, the course action supplemental form, course
action summaries, new degree requests, degree modification
requests, degree sheets and an overview of SharePoint.
Presentations will provide step-by-step guidance on how to complete
the forms and why the information is requested. Discussions will
also include timeframes and how the information is dispensed and
administered throughout the university.
The course action form was modified in Fall 2008 and no other
version of the course action form will be accepted.
