1. Designate one person to coordinate your team uniforms
2. Print out and record your players' sizes on the form below. Take your completed order form to the
store listed on the team flyer, or call to make arrangements over the phone - Please Fill Out All
Grey Boxes Below (example provided)
3. Visit your local store to view sample uniforms (if preferred)
4. Pick up uniform samples for team sizing (if required)
5. Have your team name and/or logo ready when placing your order (your local store can assist you
in logo creation if needed)
6. Allow a minimum of two weeks for order processing. Please ensure that your team order is placed
at least two weeks before you need to collect your team uniforms
7. Collect money from team parents- check with your store before placing your order to familiarize
yourself with their deposit requirements
8. Collect and pay for all team uniforms when complete
9. Hand out your team uniforms to the players
