          Jay School Department                                    Project: Substance Abuse Counselor
             1. DESCRIBE THE PRIORITY ISSUE THAT THIS TITLE IV-A FUNDED PROJECT WILL ADDRESS:

                Priority Issue #1: The percentage of students at Jay High School (66%) who inidicate that there is a
                significant problem with substance abuse (e.g. drugs and/or alcohol) among the students in their
                school. Priority Issue #2: The lack of available counseling services that students may access in a
                timely manner without financial considerations.


             2. THIS PROJECT WILL EMPLOY AN "OTHER ALLOWABLE" PREVENTION STRATEGY:
                (By selecting this type of prevention strategy you are automatically applying to the State for a waiver to
                use your Title IV-A funds for "non-model" prevention programs, services, or activities. If your project is
                approved you can consider that the waiver has been granted.)

                    Student Assistance Programs

                Counseling, mentoring*, referral services, and other student assistance practices and programs.
                Expanded and improved school-based mental health services related to illegal drug use and violence,
                including early identification of violence and illegal drug use, assessment, and direct or group
                counseling services provided to students, parents, families, and school personnel by qualified
                school-based mental health service providers. Also includes assistance provided by qualified
                school-based mental health services providers and the training of teachers by school-based mental
                health service providers in appropriate identification and intervention techniques for students at risk of
                violent behavior and illegal use of drugs. *Includes mentoring programs that encourage students to
                seek advice from, and to confide in, a trusted adult regarding concerns about violence and illegal drug
                use (For example, this would be the applicable "Other Allowable" strategy category that you would
                select for a Substance Abuse Counselor or Student Assistance Team-type project.)

             3. JUSTIFY THE USE OF THE SELECTED PREVENTION STRATEGY TO ADDRESS THE PRIORITY
                ISSUE:

                This is the third year that the Jay School Department is asking for funding to support a Substance
                Abuse Counselor. Title IV-A funds allowed high school students to have access to services one day
                each week during the 2005-2007 school years. As a result of this project, the following services were
                available:

                - Chemical Dependency Consults/Assessments
                - Ongoing counseling services
                - Assistance with referrals to outside agencies
                - Administration of the Maine Youth Drug and Alcohol Survey (MYDAUS)

                During this past year, twenty-one students at Jay High School have accessed services. Sixteen
                students received ongoing substance abuse treatment and five students were referred to outside
                agencies for treatment. Of the sixteen students who received ongoing treatment, fifteen remained in
                the school setting. Thirteen of those students maintained passing grades and will advance to the next
                grade. We are requesting the continued funding of this strategy to enable our system to continue to
                offer these services to students.

             4. DESCRIBE HOW THE SELECTED PREVENTION STRATEGY WILL BE IMPLEMENTED:

                Individual Counseling and Referral Services will be provided at Jay High School by a licensed
                Substance Abuse Counselor. Students may access services thorugh self-referral, parental referral or

               referral from the Student Assistance or Pupil Evaluation Teams. Referrals relating to school
               suspensions or other disciplinary actions will be made by school administrative staff.

             5. ESTABLISH ANTICIPATED ATTITUDE AND/OR BEHAVIOR CHANGE OUTCOMES FOR THE
                PROJECT:

               We anticipate that a minimum of twenty students will access substance use counseling/referral
               services on-site at Jay High School in the 2006-2007 school year. Of those students, we anticipate that
               80% will develop skills to address their addictions and improve their functioning in school. An action
               plan will be developed for each student. The counselor will utilize a tracking tool that will effectively
               measure attitude and/or behavior change for each referred student. Success indicators will include:
               1. Changes in attitude and/or behavior.
               2. Ability to stay enrolled and maintain passing grades.
