Scenario: Two dialysis nurses are going to do an abdominal assessment to look at a peritoneal dialysis site on a 60
yr old male who does peritoneal dialysis at home, who has just been admitted with abdominal pain. He is NPO, has
an IV running and a PCA with morphine. He is in quite a bit of pain, but the PCA seems to be helping. You arrive to
the patient’s room knock, introduce yourselves, explain what you will be doing, position the patient, evaluate the
abdomin thank the patient and before you leave the patient asks if you would be able to help him charge his cell
phone……….

1 KNOCKS before entering patient room. (Pauses before entry to listen for

2 ACKNOWLEDGES the patient by smiling and calling out his name, giving good

ASKS him his preferred name and adds it to the whiteboard if needed.
SHOW EMPATHY AND CONCERN FOR THE PATIENT: Ask how they are
doing and respond appropriately based on the patient’s response. If the patient
is known to you discuss situation appropriately.
USES HAND GEL OR WASHES HANDS says I am washing my hands or using
hand gel for your safety and mine. This gel is available for you or your family to
use as well.
PULLS CURTAIN for their PRIVACY (Key Word)
3. INTRODUCES self and manages up own skill set. Explains who they are, what
their position is, and why they are there.
CHECKS AND CONFIRMS PATIENT identification: Ask them to state their
name and birth date. Let patient know you are checking this for their SAFETY
(Key Word) to assure you are assessing the right person.
4. DURATIONS: Lets patient know how long it will take to do this assessment.
EXPLANATION: Explains the portable x ray (or ultrasound) and that will be
needed.
LETS THE PATIENT ASK QUESTIONS: What questions do you have before
we get started?
Assists the patient into position for the assessment: Show concern for
COMFORT (KEY WORD).
PERFORM THE ASSESSMENT:
5. MANAGES UP: (Physician, Nurses, Dialysis Staff, Hospitality, other care
providers, etc)
6. Assists the patient back into a COMFORTABLE position and assure belongings
and call light are left within reach.
7. THANK YOU: Thank the patient/family for their time and EXPRESSES
APPRECIATION to them for their cooperation and communication
Ask if there is ANYTHING ELSE HE/SHE CAN DO for them before ending the
interaction. (Patient asks if you would be able to help charge his cell phone—
respond appropriately (MANAGES UP Hospitality)
Encourage the patient to write down any questions that he has for his doctor.
(Offer notepad and pencil—that should be at the bedside)
